I PURPOSE AND SCOPE Background This Vessel Particulars Questionnaire asks over 700 separate questions about ship particulars and required or customary on board documents which are of a permanent or semi-permanent nature, primarily related to safety and pollution prevention. The questions are indexed by numbers and are logically grouped into separate Chapters. This Document is intended for use by entities which have a need to inspect ships, such as prospective charterers, terminal operators and port State entities. It is intended that this Questionnaire be answered by the ship Operator 1 and be made available to the inspecting party either prior to the shipboard inspection or during the course of such inspection. The foregoing will allow the inspector to have at hand complete and presumably up to date ship information which the Inspector may need or find helpful during the ship inspection.2 Instructions for Use of Questionnaire For questions requiring a yes or no, the Operator should delete either the “Yes” or the “No” in the question answer box. If a particular question or request for information is not applicable, the Operator should make a notation of N/A in the question answer box. All measurements should be given in metric units. If for any reason metric units cannot be used, this should be clearly indicated.
