EDITORIAL
Looking back at RSNA there are two terms that epitomize current developments and innovations in radiology: mobility and image fusion. Today, powerful image processing and image management solutions allow users to view images and access patient data on a multitude of mobile devices – not only in any corner of the hospital but also ‘on the go’ as long as a mobile phone network is available. But it is ultrasound technology that quite surprisingly has come up with some of the most stunning innovations in image processing and communication. Where a few years ago clinical detail seemed to be hidden in clutter noise and patients were offered but a tiny photo torn off a role of thermo paper today the sonographer can go on a virtual trip through cavities and vessels. But even better: Modern ultrasound systems correlate CT and 3D ultrasound data volumes and display findings from different modalities and from different planes next to each other on the console. Thus US-guided needle biopsies can now be performed based on CT data. This, in our opinion, is an outstanding example of modern technology that enhances diagnostic safety and patient comfort alike. Focusing more on the patient, less on technology The University Hospital of Würzburg just
migrated from Siemens SIENET Magic
to syngo.plaza, marking a further step toward optimizing workflow economy
and medical quality. It is now one of the system’s biggest users in Germany, with plans for 100 users to operate approximately 170 modalities in radiology, urology, gastroenterology, neuroradiology, and pediatric radiology.
The switch marked a major leap forward. Professor Dietbert Hahn, MD, director of the hospital’s Institute of Radiology, points out unequivocally: “we took another step toward optimizing workflow economy and medical quality with the hospital-wide introduction of syngo.plaza”. Both, time savings and quality enhancement were
major factors driving the move. Tools such as the findings Navigator, for instance: “With syngo.plaza, we can visualize measurements of tumors or other lesions in an action log. When we reopen the series later, we automatically see the images with the measurements from that time. That saves time and helps during the diagnostic process.”

Switching to the Next View in Seconds1 Another example is the Cross Reference mode, which is used for reporting CT or MRI studies. The radiologist marks a certain spatial point within a series and can then automatically locate that point in a different view. Detlef Klein, MD, senior physician in the hospital’s radiology department cites a real-world example to explain the benefits of this process: “Using the Cross Reference tool, I can switch perspectives in seconds1 to find out whether an air pocket is located within the intestine or is extraluminal.” In principle, the site’s former PACS also offered the ability to call up multiple orthogonal views, of course. “But it is significantly faster now, with just one click from the mouse menu,” Klein says. Indeed, time savings and speed are what the switch is all about. Downloading a polytrauma case with 1,600 images now takes just a few seconds1 at most, where the radiology team in Würzburg used to wait several minutes for the same process to finish in the past. Speed also affects communication within the team, as Hahn points out: “It used to take a minute and a half to load an MRI or CT series. That was unbearably long for clinical demonstrations or tumor conferences. With syngo.plaza, the images now appear onscreen right away.” Since these kinds of conferences often involve discussing images from twenty or more patients, including preliminary scans, it is easy to see how much time the new PACS saves.

In this context, Hahn lays out a simple calculation: “If it took a minute and a half to call up a study using the old PACS, and we open 400 studies per day, we can see, in purely arithmetic terms, that syngo.plaza will now help save us several hours of loading time – time that we can better use elsewhere. The image memory is large enough that we can even access the images for multiple patients at the same time.” On the topic of work speed and waiting times, Professor Matthias Beissert, MD, managing physician at the Institute of Radiology, can’t resist a slight dig at Siemens: “We had to wait a bit longer than we had originally expected for syngo.plaza,” he notes. Developing an advanced PACS simply takes time. “And a new PACS from Siemens is no exception,” he says. But jokes and critical comments aside, Beissert does ultimately have praise for Siemens as a longstanding, valued partner: “The wait was worthwhile – well worthwhile, even.” One factor in his positive overall assessment of syngo.plaza is the Smart Select function, a personalized tool that each radiologist can arrange according to his or her routines. The radiologist selects the eight most frequently used functions and places them in the “star”, where they can then be clicked directly, instead of having to use the navigation bars for access. This helps ensure that users move forward quickly, with just a few clicks, saving “mouse meters” and allowing the radiologist to concentrate completely on the image.
The fact that the data transfer took only about eight months is due to a number of factors, including the optimized management of the migration process. Those responsible for the project pinpointed a transfer speed that allowed radiologists to do their work without issues during the migration, while still keeping the preliminary studies generated earlier available for comparison purposes without long wait times. During the migration process, Siemens maintained its own office within the radiology institute to monitor the data transfer and remain available to PACS users at all times. “We always have an expert contact person nearby to help us right away with all our technical questions and issues relating to using the system,” says Klein, praising the support provided for the project.
Data Sets from 1.2 Million Scans Migrated to New PACS
So what’s the final verdict on syngo. plaza? Beissert pauses, then answers: “We are focusing less on the technology and more on the benefits that the technology brings us for our reporting. That means syngo.plaza has taken our radiology department into the future.” The new PACS is a tool that allows radiologists once again to concentrate more on the original purpose of the profession in their day-to-day practice: That is actually the best compliment syngo.plaza could hope to earn. After all, it was developed for physicians – not for engineers.
The facility in Würzburg decided on “everything in a single archive” as a solution. All of the existing data from ten years of digital radiology – 1.2 million scans, totaling 50 terabytes in all – was transferred to the new system. The department’s former PACS, SIENET Magic, has definitely been put out of commission now that the data migration is complete, and the radiologists are working exclusively with syngo.plaza.

Since the approval of the first commercial systems in 2000, digital mammography has become an accepted standard of care in breast cancer screening and diagnosis worldwide and has paved the way for the newest groundbreaking technology in this arena - breast tomosynthesis. Breast tomosynthesis is a screening and diagnostic modality that acquires images of a breast at multiple angles during a short scan. The individual images are then reconstructed into a series of thin, high-resolution slices typically 1 mm thick, which can be displayed individually or in a dynamic ciné mode.

Practical Considerations in Implementing Breast Tomosynthesis in a Clinical Setting A tomosynthesis data set virtually eliminates detection challenges associated with overlapping structures in the breast, which is the primary drawback of conventional 2D analog and digital mammography. In addition, breast tomosynthesis offers other potential benefits including increased lesion and margin visibility, help in localizing structures in the breast, a reduction in recall rates, and increased cancer detection.
Tomosynthesis has been available in Europe and other countries recognizing the CE mark since 2008. In February 2011, Hologic’s Selenia® Dimensions® breast tomosynthesis system was the first commercial system approved by the U.S. Food and Drug Administration. With the Hologic tomosynthesis system, an examination takes only seconds longer than a conventional two dimensional digital mammogram. The system was approved for use in the same clinical indications as 2D mammography including breast cancer screening and diagnosis. Prior to introducing tomosynthesis into a clinical practice, there are a number of practical considerations that
should be evaluated. Most breast imaging centers have multiple digital mammography systems, and it may not be economically feasible to immediately replace every 2D system with a Tomo-capable units. It is likely that many facilities will implement tomosynthesis in phases, beginning with one or two systems initially, similar to the pattern seen in the transition from analog to digital mammography. During this implementation phase, facilities will need to develop criteria for determining which patients will receive Tomo exams and processes to ensure efficient patient management in a mixed 2D and tomosynthesis environment. In addi-

tion to patient management planning, there are a number of other issues to be considered, including: how comparison to priors will be handled; methods to ensure radiologists receive appropriate training prior to reading Tomo exams; how reading times for Tomo exams will affect workflow; and the impact tomosynthesis will have on archiving and display requirements. These issues are discussed in more detail below, drawing from the experiences of users who have adopted tomosynthesis.
Some facilities offer tomosynthesis using the ‘next available free system’ model. In this scenario, if the room with the tomosynthesis system is free, the next patient will be triaged to the room, regardless of their breast density. Some facilities also offer full access to a tomosynthesis exam for any woman who requests the procedure. This is accomplished through the use of special after-hours or otherwise scheduled blocks of time to assure access to the system. Figure 1 shows some of the possible implementation scenarios for tomosynthesis. All of these scenarios are reasonable implementation strategies. Complete conversion to tomosynthesis All screening and diagnostic patients receive tomo exams
Use of Tomosynthesis for Women with Dense Breasts Tomosynthesis has been shown to have value in both fatty and dense breasts, but it has a greater impact for women with dense breasts. Therefore, if a practice does not have enough systems to screen all women, it is reasonable to reserve tomosynthesis for women with dense breasts. However, as tomosynthesis offers a benefit in both fatty and dense breasts, the eventual goal should be to screen all women using tomosynthesis imaging.
Use for diagnostic exams only

Use for some screening patient subgroups, such as women with dense breasts
Use of Tomosynthesis for Screening or Diagnostic Imaging Tomosynthesis has shown value in a diagnostic evaluation of a symptomatic breast. It also can be used as a screening tool to improve sensitivity and reduce recalls. Therefore, either or both indications are acceptable uses of tomosynthesis. Since diagnostic procedures often take longer than screening exams, more women per day can be accommodated on machines dedicated to screening use. This might be a consideration in situations where a limited number of Tomocapable systems are available.

These types of scenarios are illustrated in Figure 2, and represent a hypothetical clinic that switches, in the current year, some but not all of their screening systems to tomosynthesis.

The use of combo-mode imaging gives the radiologist access to a 2D current for comparison to 2D priors. This combo-mode procedure also offers some advantages in the scenarios mentioned above where a patient this year might get a tomosynthesis study and in the next year receive only a 2D study because of scheduling constraints or equipment access. In such a situation, the radiologist reading the study in the following year would have access to the 2D exams from previous combo-mode studies. Radiologist Training Proper training on the interpretation of tomosynthesis images is of critical importance, as was demonstrated in Hologic’s reader studies. To ensure access to appropriate training, Hologic has set up training classes around the world, taught by radiologists experienced in tomosynthesis. These classes cover the appearance in tomosynthesis of different density parenchymal patterns, along with benign and malignant lesions. Actual clinical cases are included in the training files, and self-assessment practice cases are reviewed and discussed to give radiologists an opportunity to test their learning. An image reconstruction algorithm creates synthesized 2D images from tomosynthesis datasets, eliminating the need for 2D screening exposures, thereby reducing both compression time and dose.
Tomosynthesis Reading Time Reading a tomosynthesis study involves the evaluation of significantly more images than a 2D mammography exam, as readers view each slice from a tomosynthesis data set containing an average of 50 or more slices. Typical slice thicknesses range from 0.5 to 1.0 mm, with more slices to review as the slice thickness is made thinner. Recent studies conclude that reading times for tomosynthesis are not unreasonable for a screening environment. The increase in the number of images reflects the increase in the information available to the radiologist, and the increased reading time is justified by the superior clinical performance.*
idly implement and support the viewing of tomosynthesis images, now that the systems are in widespread use in Europe and the U.S. In planning a site, careful consideration must be given to the size of the images. The images from a tomosynthesis study can be 5 times larger than the corresponding size of a 2D digital mammography dataset. Network speeds and PACS storage must be adequate for the increased data flow. These larger datasets are a natural result of tomosynthesis offering more information than digital mammograms.
Conclusions
Archiving and Display Tomosynthesis display and storage is covered by a DICOM standard.** Nonetheless, at this time not all PACS vendors support either the storage or display of these images. Hologic has developed, as an interim solution, a storage method whereby the tomosynthesis images are stored in a secondary capture format, which allows the storage in PACS, but for display one must use a Hologic SecurView diagnostic workstation. It is expected that PACS companies will rap-
Tomosynthesis is an exciting new technology that offers the potential for improvements in both screening and diagnostic evaluations. The improvements in clinical performance, compared to 2D mammography, are significant. Hologic’s clinical study results demonstrate that 2D mammography plus Tomo can offer either improved cancer detection rate, or reduced recall rate, or both, compared to 2D alone. These are certainly very positive results, and are much stronger than the ACRIN DMIST study results which
compared the performance of digital to screen-film mammography, and found no average difference in performance between the two technologies.*** There is a growing body of evidence that tomosynthesis has the potential to reduce the number of exposures needed for diagnostic imaging and provide other diagnostic benefits including enhanced performance in assessing tumor size and stage and more clearly demonstrating margins and extent of lesions. Future advances in tomosynthesis include CAD algorithms to facilitate the rapid identification of suspicious clusters of calcifications, development of a synthesized 2D image to reduce the number of exposures in an exam while still providing a 2D-equivalent image for ease of review, and contrast enhanced imaging for patients where access to breast MRI is limited or contraindicated. Certain implementation issues should be considered when adopting tomosynthesis, particularly in environments that will include a mix of 2D and tomosynthesis-capable systems.
