The major focus of The University of Melbourne Department of Surgery, Austin Health is the development and delivery of high quality surgical education and training. The Department provides a comprehensive surgical education program to both undergraduate and postgraduate students in parallel with the clinical surgical training delivered by the members of the Division of Surgery. In addition the department provides opportunities for scientific research in a wide variety of fields in a warm, friendly and collegial atmosphere. Four separate research groups with international repute provide excellent oppportunities for advancement in both basic science and clinical research. Researchers and students have easy access to supervisors and support and close supervision at all times. Modern facilities and easy access to transport ensure a pleasant working environment.

Welcome Message We wish to welcome all Surgical Trainees together with all members of the Division of Surgery to the General Surgery Education Program for the year 2012. This program is aimed at providing a comprehensive series of tutorials, seminars and journal club sessions to Surgical Trainees at all levels. It also provides participation to all members of the Division of Surgery through the Surgical Forums and the Annual Surgical Unit Audits. The strength of the program and its success over the past eight years has been predominantly due to the commitment and dedication by the members of the Division of Surgery who have volunteered their valuable time for the benefit of our trainees. The program continues to evolve each year based on the general attendance, impact and feedback received from both trainees and consultants. A number of changes have been incorporated in to the program for 2012 to reduce the demand on trainees and surgeons while concentrating the teaching in to manageable segments. The Saturday Surgical Seminars held once a month on Saturday afternoons have been an outstanding success with record attendances by trainees and have been open to all surgical trainees in Victoria. Another notable success was the new SET1 refresher course for trainees preparing for the surgical sciences and clinical examinations. The program will remain concentrated between April and November 2012. Some of the changes to the Surgical Education program for 2012 will demand an increased and proactive role by trainees in successfully implementing it. This includes a greater role in defining the content of the Journal Clubs as well as more emphasis on the Trainee Education Co-ordinator to ensure a seamless progression of educational activities. On Friday the 3rd of December 2011 the third Annual Austin Surgical Trainee Dinner was hosted by the trainees to demonstrate their appreciation of the time and effort imparted to them by the Surgeons and Fellows of the Austin Hospital. This will be held in 2012 at the same venue on Saturday the 1st of December. Finally, the Austin Research Prize will be held on Tuesday the 27th of December 2012 and we encourage all surgical trainees to participate. We wish to emphasize that the program is open to anyone who is interested from the Austin and other centres, including all Surgical Trainees, Unaccredited Registrars, Surgical HMOs, Interns and International Medical Graduates. The success of this program is dependent on active participation from all Surgical Trainees and staff members. Thank you to all the members of the Division who participate in the surgical education program and we look forward to another successful year.

Surgical Education Program INTRODUCTION: All Surgical Trainees are expcted to participate in the Surgical Education Program. This program provides a continuous series of educational activities for trainees at all levels. Since its inception in 2003, the program has continued to evolve and at present encompasses a wide range of topics that is intended to provide a broad knowledge base. SURGICAL TRAINEE REPRESENTATIVE: Two trainee representatives based at the Austin Hospital are chosen (usually SET4 trainees) to co-ordinate the Surgical Education Program. It will be the trainee representative’s responsibility to communicate with the relevant tutors in advance to confirm the sessions or make alternative arrangements if necessary. One trainee would be the designated contact person for SET2+ trainees while the other designated to cordinate the SET1 and NSET trainees. ATTENDANCE: Attendance is MANDATORY for all SET trainees based at the Austin Hospital. All trainees including those on regional rotations are expected to attend the Saturday Surgical Seminars. Surgical HMOs and Interns are also strongly encouraged to participate. ABSENCE: Trainees who are unable to attend a particular session are expected to inform the Surgical Trainee representative so that apologies may be noted. If trainees are unable to attend a session where they are expected to present, they must make alternate arrangements. General Surgery Trainee Representatives Since their inception in 2009 the General Surgery Trainee Representatives have played a major role in liaising between trainees and their teachers and ensuring the smooth running of the Surgical Education program. Given the increasing number of trainees based at Austin Hub two trainees will be assigned the role to be shared between them. The trainee reps will also play an increasing role in determining the content of the sessions, in particular they will be instrumental in the running of the journal clubs. In addition to their role in coordinating the education sessions they will also mentor junior surgical trainees and act as a link between trainees and their respective supervisors. 0730 – 0830 (Light Breakfast at 0715) Wednesday Lecture Theatre, Dept of Surgery, Level 8, Lance Townsend Building The Surgical Forum will be conducted in two parts as described below:
INVITED LECTURES (February – June) This will be conducted in the first half of the year from the 15th February to the 27th June. Speakers have been invited by the department to deliver clinical updates on current topics. A mix of internal and external speakers havs been chosen based on their expertise. SURGICAL UNIT ANNUAL AUDITS (July – November) This will be conducted in the second half of the year from the 4th July to the 21st November. Each unit within the Division of Surgery is allocated a single day for a presentation of their Annual Surgical Audit. The Austin Surgery Annual Surgical Audits provide an opportunity for all surgical units to present their annual activity, trends, outcomes and benchmarking. The unit members are encouraged to present the figures for the unit activity and the general and specific morbidity and mortality rates for the previous 12 month period. The morbidity should preferably relate to both the operation and the indication if possible (criteria auditing). All deaths should be listed. Detailed discussion of complications and deaths should be avoided as this would have been performed at the weekly surgical audits. However, instances where morbidity or mortality had resulted in a change in practice should be highlighted and discussed if necessary. If available, other data on key performance indicators and comparative data from other comparable hospitals would be appreciated. The duration of the audit meeting should be approximately 45 minutes. All units are expected to supply an electronic version of the audit to the Department of Surgery archives. (See Kim McAughtry) It would be ideal if the heads of units would encourage other participating staff to attend (apart from the unit members) to their unit audits. This may include ward, theatre and anaesthetic nursing staff, as well as staff from other departments such as physiotherapy, nutrition, etc. In addition, members of similar units from other Melbourne hospitals may be invited and encouraged to attend. The Surgical Foum (Wednesday 7.30 - 8.30am) now forms part of the protected teaching time for all surgical HMOs at Austin and thus mandatory attendance is expected.

Audit Content: The audits should display the weekly activity of the relevant unit, including operative, endoscopic and non operative cases. Morbidity and mortality should be listed with all UR numbers and relevant cases discussed in detail highlighting specific issues and any suggested corrective measures.
Audit Submission: Electronic format of the audit should be sent to Kim McAughtry at the Department of Surgery (T: 9496 5468; E:
[email protected]) to be received latest by 1400hrs on the Wednesday preceding the Audit. These will be uploaded to the computer in the Lecture Theatre ready for presentation. Late submissions will NOT be uploaded.

Audit Format: The surgical audit is presented electronically using the lectern computer and projector.
If you are unable to submit the audit in time, you may bring a copy on USB Memory Stick and physically load it on to the computer on Thursday morning. If you plan to do this, kindly arrive ahead of time (before 7.15am) to load the audit and avoid disrupting the smooth flow and conduct of the audit session.

Saturday Surgical Seminars SATURDAY SURGICAL SEMINARS: Time: 1400 – 1730 Day: Saturday (Monthly) Venue: Lecture Theatre, Dept of Surgery, Level 8, Lance Townsend Building Each surgical seminar will consist of three parts as described below: 1330 - 1400 Lunch 1400 – 1430: Operative Surgery Quiz by the panel 1430 – 1500: Medical Updates for Surgeons 1500 – 1530: Trainee presentations x 2 1530 – 1545: Afternoon Tea 1545 – 1630: Case presentations & quiz by Surgical Fellows 1630 – 1700: “How I do it!” by the chairperson The aim is to provide to an interactive monthly session on a Saturday afternoon to enable trainees from metropolitan and regional as well as interstate rotations to participate.
The aim of each trainee presentation should be to present a concise summary of the topic which should include an overview of current management principles, recent advances and future directions. Controversies in the subject should be discussed and audience participation encouraged. Presenters are encouraged to question trainees in the audience and vice versa. An essential part of the seminar should be a detailed study of the literature encompassing at least the past five years. Please do not attempt to present a text book based account of the subject! Trainees are advised to contact the chairperson of the session well in advance to discuss the direction and focus of the seminar. You should expect to begin preparation at least two to three months in advance. All trainees except those posted interstate are expected to attend. Given the monthly frequency of the sessions, we expect trainees to be able to adjust their on call commitments in order to attend. Interstate trainees are strongly encouraged to attend where possible.

Each Surgical Seminar will consist of the popular operative surgery quiz by the panel members. This will be followed by a consultant physician delivering a formal presentation on a very relevant non-surgical issue. Two trainee presentations of 15 mins each complete the first session. After s short break the surgical fellow relating to that subspecialty or one of the SET5 trainees would present a number of case studies related to the topic discussed. The session ends with a presentation (preferably video) by the chairperson.
All non Austin surgical trainees are also most welcome. 27

Surgery and Scientific Research: Why do it? Research Opportunities & The Academic Research Program Research activity has progressively increased in importance to the surgical trainee over the past years as emphasis has increased on a scientific approach to surgical problems. Increasing demands are made on trainees to conduct, present and publish on surgical research not only as a component of their accredited training but also to increase their career opportunities and successful applications for future fellowships. The Department of Surgery continues to strongly support research undertaken by trainees either as full time research higher degrees or on a smaller scale during their clinical tenure. Trainees at all levels from HMOs to advanced SET trainees are encouraged to seek opportunities early in the year to begin work on at least one research project. If you have a particular interest in a subspecialty, then feel free to approach members of that unit for a potential research project. If not, the Department of Surgery will be able to provide a number of options for small, relevant clinical studies based on our ongoing prospective data collection service. The aim would be to present the results at the Austin Research Prize presentation and subsequently to have it published in a relevant peer reviewed journal. Research for Surgical Trainees: Surgical trainees interested in an academic career will have strong backing from the department to enrol full time in one of the Research Higher Degrees. In addition we also support all surgical registrars, in both accredited and unaccredited positions, to undertake serious research concurrent with their clinical work. These are mostly small project based clinical studies involving data from our clinical databases, hospital medical records and where necessary, tissue from the Victorian Tissue Bank.

Research Higher Degree (RHD) Program: The Department of Surgery strongly encourages the undertaking of RHD by research (PhD, MD M Surg and MSc). The main focus of such research is related to hepatobiliary and pancreatic disorders, organ transplantation and regulatory peptides. Students from both the medical and biomedical fields are welcome. There is also strong international interest from regional centres in this program leading to international post doctoral students undertaking this course. Currently active research projects include the treatment of liver metastases by vascular disrupting agents, macromolecular drugs and hyperbaric oxygen, the role of liver regeneration in tumour growth, the role of microcirculatory disturbances and hyperbaric oxygen in severe acute pancreatitis, xenotransplantation and the role of regulatory peptides in the development of liver, colon, prostate and kidney tumours. In vivo and in vitro models for each of these projects are well established in the Department. Clinical material and tumour banks are extensively used. Details are available in our “Pathways to Discovery” booklet which can be obtained from the office or downloaded from our web site. Research for Clinicians: The philosophy in the Department of Surgery encourages scientific research at all levels in clinical, basic science and translational aspects. We also endeavour to establish strong links with other departments to develop collaborative research including the Peter McCallum Institute, and clinical departments such as Anaesthesia, Radiology, Oncology and Intensive Care. Detailed surgical databases have been developed for both general surgery and specialist HPB disorders. Data collected within these provide a solid base for developing short term clinical studies. All such projects will aim to have the results published in a peer reviewed clinical journal within a year of completion. Research for Junior Hospital Doctors: Recognizing the need for small project based clinical studies for junior hospital medical officers (Interns, HMO 2 and 3) we offer a range of clinical and mixed projects. These are available to anyone interested in developing a future career in surgery. They are structured to be completed within an year to enable a full time junior doctor to complete without enrolling for a formal research degree. Expectations and Outcomes of Research: The departmental philosophy supports the timely completion of all research undertaken by providing the necessary facilities, mentorship, technical expertise and funding support. In general everyone who undertakes research activities in the department will be expected to present their results at local, national or international meetings and be strongly encouraged to publish in peer reviewed journals. All full time research students are expected to present at the annual Austin Medical Research Week while clinical research undertaken by surgical trainees and junior hospital doctors will be encouraged to be presented at the annual Austin Surgery Research prize. 35
Austin Surgery Training & Research Requirements All Austin Surgery Trainees undertaking General Surgery Training are expected to achieve the following: 1. Publications in peer reviewed journals: A minimum of one but preferably an average of two publications when they present for their FRACS exams. Trainees should have contributed to at least 40% of the publication regardless of authorship level. 2. Austin Research Prize: All Austin Surgery Trainees are expected to present at the Austin Research Prize at least once in their five year training period. The research requirements for presenting for the final FRACS exam are at present being reviewed and would continue to change as the emphasis on research as a tool for education and understanding clinical issues increase. The department of surgery at the Austin Hospital and the major general and specialist surgical units offer a wide variety of research opportunities ranging from small clinical studies to full time research as described in the previous pages.The best period in which to undertake reasonable research is in the early years of SET training particularly in SET1-3 as the emphasis of the trainee changes in SET4 towards exam preparation. It is important to consider that converting practical results in to a publishable manuscript takes time. Time to publication may very well take 18 - 24 months depending on the quality of the research, the standard of the journal and the number of revisions required all of which must be taken in to account when planning their research. There are a number of opportunities to present your research at national or international conferences in 2011 as described below: RACS Annual Scientific Congress: 7 - 10 May 2012, Kuala Lampur, Malaysia. The Annual Scientific Congress (ASC) is the College’s major educational annual activity and is the largest multi-disciplinary surgical meeting held in Australia and New Zealand. Eligibility Criteria: Trainees belonging to a training scheme within the Division of Surgery in ALL surgical specialties ( all SET and Non-SET registrars, Surgical HMOs (PGY2 & 3) as well as Advanced Trainees in Anaesthesia, Radiology, Emergency, Intensive Care & Pathology at Austin Health are invited to submit abstracts relating to original scientific research. Exclusion Criteria: Previous winners of the Austin Surgery Research Prize and interns. Presentation Format: Oral presentation judged by a panel of adjudicators. Presentation Timings: Each candidate will be allocated a maximum of 15 minutes for their presentation. Candidates should aim to limit their talk to eight minutes after which there will be seven minutes of question time. A warning bell will be rung at eight and fourteen minutes in to their presentation and a final bell at fifteen minutes to mark the end. Prizes: The trainee with the best presentation as judged by the adjudicators will be awarded a gold medallion with ribbon by the Johnson & Johnson representative at the end of the proceedings. Subsequently they will have to be returned so that they may be mounted on a special frame as shown along with an inscription. The winners name will also be added to the Honour Board in the department of surgery. All presenters will be awarded certificates of attendance.

Masters in Surgical Education The Graduate Programs in Surgical Education are a new suite of qualifications offered by the University of Melbourne through its Department of Surgery and Medical Education Unit in partnership with the Royal Australasian College of Surgeons (RACS). This suite of programs addresses the specialised needs of teaching and learning in a modern surgical environment. The programs’ content recognises the unique challenges that characterise the clinical settings and advanced technologies that are increasingly important in surgical training. Effective teaching skills are essential attributes for educators responsible for training the next generation of surgeons in the complex sets of skills required for safe surgical practice. The programs allow surgeons to gain formal skills in teaching and educational scholarship. Program delivery includes core and elective subjects delivered through a combination of workshops, distance and online delivery modes. The program will expose students to a range of educational methods, which in turn increase program flexibility, allowing these studies to be balanced with clinical commitments. The content reflects critical issues in the broader education community together with specific challenges for surgical education – the role of regulatory bodies, balancing clinical service with training, ethical imperatives for simulation-based education, safer working conditions including safe hours and more. Students have the option of enrolling in the Graduate Certificate, Graduate Diploma or Master of Surgical Education based on their own educational needs. Program Structure: Program delivery includes core and elective subjects delivered through a combination of workshops, distance and online delivery modes. The program will expose participants to a range of educational methods, which in turn increase program flexibility, allowing these studies to be balanced with clinical commitments. Entry Requirements & Applications All applications for entry will be assessed by the Selection Committee who will evaluate the applicant’s ability to successfully pursue the program. The following criteria specify entry requirements: 1. A Bachelor of Medicine/Bachelor of Surgery or equivalent qualification plus, 2. At least two years of relevant clinical work experience. It is important that students have an appreciation of clinical practice before they commence studies for this course. 3. Meet the English language requirements of the University of Melbourne. International Medical Graduates must also meet the English language standards required by the Medical Board of Australia (an IELTS score of 7.0 in each of the four components - listening, reading, writing and speaking). The Selection Committee may conduct interviews and tests and may call for referee reports or employer references to assist in the selection process. This is an event hosted by the Austin surgical trainees in appreciation of the education and training given them by the surgeons at the Austin Hospital. The initial event was held in 2006 and after a hiatus of two years recommenced in 2009. The Austin Trainees conducted a very successful dinner event on Friday the 3rd of December 2010 which for the first time included surgeons from many of the attached training hubs. Changes for the 2011 event included the timing to be on a Saturday night which resulted on more than 70 attendees including invited surgeons and trainees from regional and interstate centres. The event provides an informal setting for trainees and surgeons to meet. In particular trainees were able to meet in person some of their consultants for the following year. This event has progressively expanded both in scope and quality and will be a perpetual event in the future.

University of Melbourne Department of Surgery Web Site The clinical, educational and research activities of the Austin Department of Surgery is supported and complemented by a comprehensive educational and informative web site. The site contains all the information required for trainees interested in a career in surgery, ranging from the Surgery education program, research opportunities, presentations and statistical tools. Trainees are advised to drop by and browse the content to help them in the many aspects of their future career. Some of the more notable and useful sections of the site are described below: Education: This section leads to a number of links. Medical undergraduate education lists the departmental commitments as well as links to the lectures in surgery series for medical students. All powerpoint presentations are available as pdf files for download. The Specialist surgical training section details the Surgical Education Program, Austin Research Prize and Research opportunities. The CME section leads to the annual HPB symposia which contains all the presentations of the past years. Research: The basic science section details the three major research groups in the department. The research opportunities section details all the possibilities available for students and graduates from both the science and medical streams. SSSA: The department inaugurated the Students Surgical Society of Austin in collaboration with The University of Melbourne and the Austin Hospital to encourage students towards a career in surgery. Events: Links to major events including research related events as well as local, national and international conferences.

Departmental Publications The Department of Surgery publishes a number of regular annual publications relating to surgical education, research and training. These are aimed at providing all relevant information to trainees and surgeons alike and encourage participation in the educational and training activities. They may be readily downloaded from the departmental web site or may be requested as hard copies from the departmental office. Pathways to Discovery: This is the flagship publication for the research arm of the department. Printed as an 88 page A4 full colour booklet it contains details of all undergraduate and post graduate research degrees offered by the department along with information on research groups and research projects available to science students and surgical trainees to undertake. Although a large number of research projects are described in the book that is only the tip of the iceberg when it comes to research opportunities.
Austin Research Prize Book: This booklet is the official publication for the Annual Austin Research Prize in Surgery & Anaesthesia. It is published with all the abstracts being submitted in that year along with the abstracts of the winners from previous years. Printed as an A4 full colour booklet, it is made available on the day of the Austin Research Prize Presentation.
Austin Surgery Education Program: This is printed as an A5 sized 44 page full colour booklet for ease of storage. It contains all relevant details about the surgical education program, surgical rosters and details of departmental activities.
