PERIODIC INSPECTION OF PROCEDURES

1. DESCRIPTION Confined space - a space or work area not designed or intended for normal human occupancy, having limited means of egress and poor natural ventilation, and/or any structure, including buildings which have limited means of egress. This SOP is provided to assist our worker entering a confined space where hazards are associated with. However, this is only a guideline with information pertaining to entering a confined space. 2. POTENTIAL HAZARDS Drowning, shortage of oxygen and presence of harmful gases can cause fatality or severe injury 3. LOCATION: Confined spaces identified at entire IFP site 4. PRE-ACTIVITY MEETINGS/CO-ORDINATION: The contractors and/or the representative designated to supervise the job will inform the Kewaunee Construction Manager /Site Engineer & the Site Safety Officer. The person contractor personnel designated to supervise the job, will have the overall responsibility of all the safety measures being implemented 5. WORK PERMITS: Contractors should mandatorily take a Confined Space Work Permit signed by the Construction Manager, Safety Officer after reviewing the site conditions, safety measures taken and contractor equipments on site. 6. SAFE WORKING PROCEDURE: Contractor will ensure that the following check points are followed as applicable where the activity is being done:  The confined space drained, gas freed, flushed & all incoming & outgoing lines rendered safe by all suitable means  The confined space left open for fresh air circulation before entry (Min 2 hrs before entry after opening the confined space)  The surrounding area checked & barricaded.  The drain, pit, sewer & floor opening/ confined space checked for presence of toxic & flammable vapors.

 The level of oxygen inside the confined space checked & > 19.5%.  Proper ventilation & lighting available/provided.  The standby person is trained & deputed outside to watch the activity.  The equipment isolated & LOTO applied.  The proper scaffold / ladder provided & tied at the top.  The 24-volt lamp used for lighting.  The safety harness used & lifeline anchored above the work level.  The required Personal Protective Equipments (SCBA or supplied air line hose mask (if necessary), leather/cotton hand gloves, safety glasses, safety shoes & Safety harness or belt) provided.  The employees working in confined space are trained & training records are given to EHS before the start of work.  The SOPs & Training records will always be available at the work place. 7. PERSONAL PROTECTIVE EQUIPMENT: PPE will be provided based on the hazard assessment. However the following PPEs will be used for carrying out work in confined space: SCBA or supplied air line hose mask (if necessary), leather/cotton hand gloves, safety glasses, safety shoes & Safety harness or belt 8. TRAINING: The operators are trained in the process as well as the High risks involved. They are trained on the High-risk activity procedures. 9. EMERGENCY PROCEDURE:  

In case of any emergency contractor will immediately inform the Site Construction Manager or if he in unavailable, the Site Safety Officer. Based on the emergency situation, further necessary action to be taken keeping the Kewaunee site management in loop.

10. PERIODIC INSPECTION OF PROCEDURES: The work instructions and safety procedures are reviewed annually.
