STATEMENT OF STANDARD OPERATING AND SAFETY PROCEDURES Safety Standard 28 is Schlumberger policy to perform jobs only where risks are acceptable. Since apparently low risk pumping jobs may present large risks if the well becomes out of control, pre job risk evaluation is required to determine if a job may be safely performed. This Standard Operating and Safety Procedure provides a system to evaluate and classify risk, defines the notification of line management that is mandatory when risk exceeds certain levels, and sets out training requirements for persons involved in pressure management operations. Additionally this Standard Operating and Safety Procedures specifies minimum distances from the wellhead for rigging up when well control risks are present.

OBJECTIVE To prevent personal injury or damage to equipment due to well blowouts during land pumping operations.

SCOPE This Standard Operating and Safety Procedure applies to land pumping operations. (Offshore Schlumberger well control emergency plans are integrated with the customer’s emergency and well control plans. Offshore, contact Schlumberger Management if Schlumberger plans are not integrated with the customer’s or if there are questions about the customer’s plans.)

POLICY Risk Classification: Schlumberger will perform jobs only where operational risks are acceptable in magnitude. On jobs where the well is out of control or where there is the possibility that the well will become out of control, operational risk must be classified using the methods in this standard. Minimum Distances: In Code Black situations the rig up must be a safe distance and at least 300 feet from the wellhead. Schlumberger personnel must remain at least 150 feet from the wellhead. In Code Red situations the rig up must be a safe distance and at least 100 feet from the wellhead. Use caution when approaching the wellhead. In Code Yellow situations the rig up must be at a safe distance and at least 100 feet from the wellhead. Use caution when approaching the wellhead. In Code Green situations the rig up location must be a safe distance, recommended to be at least 60 feet from the wellhead. The provisions of all Sections of the Well Services Pressure Pumping Standard Operating and Safety Procedures (located: http://quality.sugar-land.dowell.slb.com/qhse/slpm/index.htm) still apply. Notification and Approval: In Code Black, Code Red or Code Yellow situations the Location Supervisor must notify Schlumberger management of the job circumstances, and gain approval to perform the job, prior to rigging up, or as soon as possible after risk conditions have changed. Location Supervisor: Each job shall have a Schlumberger person designated in charge and known as the Schlumberger Location Supervisor.

Pressure Management Training - Level 1: All operations personnel in land locations are required to undertake Pressure Management Training – Level 1 provided by the Schlumberger Pressure Management Awareness Training Package. Pressure Management Training - Level 2: Locations performing pressure management jobs are required to have at least one person with current Pressure Management Training – Level 2. Pressure Management Training – Level 2 consists of Well Control Training by a industry recognized trainer; at least a three day school with a one day refresher every third year. Customer Safety Policies: Schlumberger will comply with customer safety policies and procedures as long as those policies and procedures meet or exceed Schlumberger Standards and Standard Operating and Safety Procedures. 5.

Schlumberger personnel must remain at least 150 feet from the wellhead; including when making the treating iron connection to the well. The customer or personnel from the well control company will perform operations that are within 150 feet of the wellhead.

Begin this process when a pump rental job is called in. Gather job information. Use the Pressure Management Data Sheet. Answer the questions on the Conditions Questionnaire and Preparation Questionnaire; circle the correct answers to the questions. Determine the Condition Level (A, B, or C) by noting the highest ranked column containing a circled answer. (Example : even a single circled answer in column C fixes the Hazard at Level C) Determine the Preparation Level (in the same manner as the Hazard level). Determine the risk code on the Risk Code Table. Select the square corresponding to the Hazard Level and the Preparation Level determined on the questionnaires. ~~~~~~~~~~~~~~~~~~~~

5.7 Responsibilities Operations Management Responsibilities - ensure that all operating personnel receive Level 1 training on this Standard Operating and Safety Procedure. - ensure that the location has at least one person Level 2 trained person available before undertaking a Pressure Management job at Code Yellow, Code Red, or Code Black. Sales, Logistics and Support Personnel Responsibilities - obtain as much information as possible, as early as possible. - notify line management when the risk code Yellow, Red, or Black. - ask questions and seek additional information in order to quantify the level of risk. - communicate job information to management and to the Location Supervisor so that jobs can be done with the lowest possible risk. Location Supervisor Responsibilities: - Quantify the level of risk, using the approach in this standard. - When the risk code is Black: Notify management and obtain approval. Rig up at least 300 feet from the wellhead. Do not allow Schlumberger personnel to approach closer than 150 feet from the wellhead. Perform the job being alert to changes that increase risk. - When the risk code is Red or Yellow: Notify management and obtain approval. Rig up at least 100 feet from the wellhead. Perform the job being alert to changes that increase risk. - When the risk code is Green: Perform the job being alert to changes that increase risk. - Plan and perform jobs in a manner minimizes risk 6.
Exemptions Exemptions to this standard require the approval of SDC Management and Technique via the exemption procedure defined in OFS QHSE Standard S10.

Definitions Fall Radius - The horizontal distance from the well head equal to the total height of the rig, where total height is measured from ground level to the upper most point of the rig. Kick - Unwanted entry of water, gas, oil, or other formation fluid into the well bore. Location Supervisor - The person designated in charge of Schlumberger operations on a job location. The Location Supervisor has overall authority for Schlumberger operations as well as responsibility for communications with the client and with the third parties contracted by Schlumberger. Site Survey - Survey of the job to identify hazards so that risk can be minimized. Well Control - The practice of controlling well pressure. Maintaining the well in a state of equilibrium, with the formation pressures contained either by well bore hydrostatic pressure (primary) or by mechanical pressure control equipment (secondary), such as BOPs, plugs or packers. Well Control Equipment - Equipment used to maintain control of the well; BOP, Hydril, flanges, threaded connections, valves, etc. Well Control Plan - A sequence of actions designed to control pressure kicks and return the well to normal hydrostatic control.

The People: Sales Engineer Schlumberger Operations Manager Area Asset Manager Schlumberger Location Supervisor Company Man
: Sam Speaker : Billy Wilson : Phil Thompson : Harold Smith - GSS : Chester Long
The Story: It’s 2:30 PM on Friday when District Sales Engineer, Sam Speaker, receives the call. The largest independent oil operator in the area needs a pump unit to help kill a well being drilled 35 miles south west of the district shop. The well has taken a kick and is shut in with the bit near bottom. Sam phones the dispatcher, and contacts Operations Manager, Billy Wilson, telling him that a Pressure Management job is unfolding. Sam makes a call to the drilling rig office, seeking more information. In spite of repeated attempts, the phone is not answered. 2:35 PM - Billy Wilson contacts Schlumberger Area Asset Manager Phil Thompson advising that a Pressure Management job is being mobilized and that details will follow. 3:00 PM - GSS Harold Smith, one assistant, and a double pump cementer carrying 120 feet of treating iron, depart for the jobsite. Harold has been to a well control school, and his assistant’s QHSE training is up to date, including Standard Operating and Safety Procedures for Pressure Management Operations (this Standard Operating and Safety Procedures) 4:00 PM - Arriving on location GSS Harold Smith surveys the situation. Careful not to miss important details he uses the Pressure Management Data Sheet and the Pressure Management Planning Checklist. He learns that the BOPs, rated at 5,000 psi, are in top shape and were tested just yesterday. There is 3,900 psi on the annulus and 3700 psi on the drill pipe. The rig crew has just finished mixing 200 bbl of 15 lb per gallon kill mud. There is no mud engineer on site. 4:20 PM - Company man Chester Long, well known in the area for fast drilling times, is in a hurry. “Rig up and get on with it” advises Chester, “I want to be cementing casing by this time tomorrow.” Chester requests that Schlumberger pump kill mud at 20 bpm and 4,500 psi down the drillpipe, proposes a location for the Schlumberger Pumper adjacent to the rig fuel tanks. While there is a clear path to the road, the rear of the Schlumberger pumper would be 75 feet from the wellhead. The kill program has been detailed by Chester using templates from a well control school.

That will put my people and equipment just over 150 feet from the well head. Rigging up there will change my situation to Code Yellow.” After some discussion, Chester agrees. 4:25 PM - Harold makes the call to Billy Wilson. “Billy”, he says, “we are in a Code Red situation here. 5,000 psi stack with 4,100 psi on the annulus. In order to drop to Code Yellow I need a second pumper with more treating iron. “ Billy dispatches a second pumper with iron on board. Billy and Harold agree that the situation is now Code Yellow and that the job can go ahead. Billy grants approval. Until the job is completed Harold will call in periodically, advising if or if not the situation code has changed. 4:35 PM - Billy Wilson calls Schlumberger Area ASSET Manager Phil Thompson to tell him that the Pressure Management Job is a Code Yellow. Example - The Story – Continued 4:45 PM - Harold and assistant begin rigging up. Harold visits the rig floor only once, witnessing the connection of the Schlumberger treating line to the standpipe manifold. 5:30 PM - The second double pump cementer and the needed treating iron arrive on location. 6:00 PM - Chester and the entire rig crew are present as Harold holds and information packed tailgate meeting. Harold uses the Pressure Management Operations Checklist as a guide during the meeting. Chester confirms his kill program pressure predictions and goes over the assignment of the rig crew. Contingency and emergency plans are discussed until Harold is sure everyone understands. 6:45 PM - The kill program begins. 9:00 PM - The kill program is completed, without incident; the rig pumps take over at a normal circulating pressure.
