• General (knocked over dumpster) • indirect cholinomime1cs (ACh mime sign in the background) • blocks the degrada1on of ACh in the synapse, increasing the concentra1on of ACh • -s1gmine is the usual suffix • similar effects to cholinomime1cs in eye, GI, respiratory tract, and urinary tract • enhance effects of ACh at NMJ motor end plate (brick wall that looks like skel muscle with electrical plug plate) • used in the treatment of Myasthenia Gravis ("gravis" graffi1 on wall) ○ only treat symptoms of MG, immunosuppressants affect an1body levels • Pyridos1gmine (lady with community "pride" jacket cleaning the graffi1) • used in long term treatment of MG • Neos1gmine (neon sign) • not as commonly used • treats urinary reten1on (bladder hose) • Edrophonium (phone booth) • "quarters only"- pyridos1gmine, neos1gmine and edrophonium are quaternary amines and do not have CNS effects • used in MG diagnosis (tensilon test- tense phone wire) ○ short dura1on of ac1on. If edrophonium improves symptoms it means either MG or not enough AChesterase inhibitors for pa1ent. • General • PCT is highly permeable • majority of concentra1on of Na, K decrease in PCT • bicarb reabsorp1on happens here • Na/K ATPase on basolateral membrane (vending machine) • Na/H+ exchanger on apical membrane (peanut guy) moves Na out of lumen, H+ into lumen • bicarb is reabsorbed in PCT lumen (biCARb), combines with H+ to form carbonic acid (H2CO3) ○ keeps pH levels in balance ○ Carbonic anhydrase converts carbonic acid into H2O and CO2 § enters the intracellular space § water is reabsorbed in the PCT § H2O and CO2 are combined again by carbonic anhydrase to carbonic acid § carbonic acid disassociates back into H+ and bicarb
