1. Fire Fighting Equipment Shortage It was observed that only four fire extinguisher of DCP-25 Kg were available at site which are insufficient to deal with emergencies as there are a lot of flammable substances are present at site. Consequence Storage of fire fighting equipments raises the issues of fire spread and damage to workers as well as property. 2. Hygienic Hazard It was observed that drinking water used at site for drinking purpose was contaminated and storage facility was dirty and poorly maintained. Consequence Poor quality of drinking water will result in occupational diseases. 3. Fire Hazard It was observed that non-explosion proof motor was being used for crude oil pumping. Consequence Non explosion proof motor can result in fire or explosion as crude oil is highly flammable liquid. 4. PPEs Non Compliance It was observed those workers were performing their duties without safety helmet and Coveralls at plant area. Consequence Non compliance of PPEs will result in human injuries.

Immediate: Provide sufficient nos of DCP-12 Kg, DCP-25 Kg, and DCP50 Kg fire extinguishers. Medium: Fire fighting training to be provided to all the staff. Long Term: monitoring of inspection of extinguisher regularly. Immediate: Purified water bottles should be provided. Medium: A proper water purification unit to be installed for the provision of pure water. Long Term: Continuous monitoring and inspection to be carried out to find out any further short coming. 5. Fall from Height It was observed that 03 workers were working maintenance job at height approximately 25 feet without any safety harness. Consequence Work at height without safety harness can result in fall from height as well as result in injuries.

Immediate: Stop the job Immediate and provide safety belts and harnesses. Medium: Arrange training 3 Week which program focusing hazard of working at height and their precautions. Long Term: Regular 2 Months monitoring regarding compliance and effectiveness of training.

6. Electric Hazard It was observed that loose and open electric connections were found near 3 phase separator. Consequence Loose or open electric connections are not only causing of electrocution hazard but also source of igniting leading to fire.

Immediate: Secure the Immediate loose connection and joints properly. Medium: proper cable 1 month trays required to be routed al the cables. Long Term: prepare SOP 2 months for safe lying of cable and their continuous monitoring.

7. Slip/fall Hazard It was observed that oil spillage and broken platform were found in plant area. Consequence Oil spillage and broken platform can result in slip and fall injuries, cuts and fractures.

Immediate: proper clean Immediate the oily surfaces, barricade the broken platform area. Medium: Arrange repair of 1 Weeks broken platform. Long Term: Arrange adequate undergrounding of 2 Months all pips or run all pipes through channels.

Health Hazard It was observed that welding job was carried out at workshop whereas workers standing nearby were without safety goggles and safety mask. Consequence Welding fumes are very hazardous to health resulting in respiratory diseases; also welding flame can result in eye arching. 9. Un calibrated LEL detection system It was observed that fixed LEL detector installed at plant area was not calibrated. Its calibration due date was expired but yet it was not calibrated. Consequence LEL detectors provide early warning in case of LEL leakage. As detector is not calibrated hence it is not reliable. So in case of LEL leakage it can be disastrous. 10. Chemical Hazard. It was observed that different chemicals having different nature like glycol, MEA and Sun oil were stored at same location without partition/segregation. Consequence All chemicals have different nature and one of them can act as ignition source for other which might be cause of fire. 11. Welfare Facilities. It was observed that unavailability of welfare facilities such as staffs were heating their meal on steam lines. Consequence Heating meals on steam lines might raise the issue of burning.

Immediate: Calibrate the Immediate LEL detectors. Immediate: segregate all the Immediate chemicals according to their properties. Medium: place MSDS on all 1 Week chemicals. Long Term: arrange partition between each chemical.

Confined Space Hazard

___________________________________________________________________________ Introduction: This report follows an inspection of Balkassar Field of Pakistan Oil Fields on January 05, 2014 in order to meet the requirements of the Nebosh IGC-3 practical application. The areas concerned with this inspection are the oil and gas production and storage facility. Also associated workshops were inspected. The premise was occupied by up to 48 staff members at any one time. Staff work was compromised of day and night shifts.

Executive Summary: In General the attitude of the Organization towards health and safety was excellent. Pakistan Oil Fields Limited has set up proper HSE policy and HSE management system was in place. Proper HSE orientation system was in place for visitors. Welfare facilities provided for workers were very good and as per requirement. However it was found that there were several areas of concern which lead to breaches of the Health and Safety regulations. The health and safety of the employees who are using the premises may also be at risk if some of these concerns are not addressed immediately. Failure to address these issues may lead to attention and possible enforcement action from statutory bodies. A detailed survey report is attached. Some points are required to be addressed which need prompt attention to avoid any accident/mishap.

Main Findings of Inspection:



Observation # 01 regarding Fire Fighting Equipment shortage

It was observed that only 04 nos. of fire extinguishers DCP-25 Kg were placed at the site where many combustible materials were scattered and were being used at the site. This quantity of DCP fire extinguisher is totally in adequate. Sir, this is against the ILO regulation of fire fighting equipments at work place. On the other hand it will not save many innocent persons because the current quantity of fire extinguishers is totally inadequate and is not enough to cope any emergency. In case of any fatality company will have to face penalties if legal notices.

Observation # 02 regarding poor hygienic condition

It was observed that workers were linking contaminated water which was store in a poorly maintained water cooler. It can cause health problem which will reduce worker’s effect can also effect overall performance. Furthermore company has to bear medical compensation cost for the individuals. It is also against ILO regulation regarding hygiene conditions at work site.



Observation # 09 regarding calibration of LEL detection system

It was observed that Fixed LEL detector installed at plant was not calibrated. Calibration due date was December 13, 2013 but yet it was not calibrated. In case of LEL leakage & Wrong detection by sensors can result in disaster as emergency siren system is connected with detectors. . Also this is against the ILO-regulations of LEL handling.



Observation # 12 regarding Confined Space Entry

It was observed that sludge cleaning was carried out in crude oil tank; one person was working inside tank without any safety harness/ lanyard for escape. Secondly no attendant was present outside tank. Worth mentioning is that it was crude tank and crude contains traces of dissolved H2S. Confined space entry without safety protection can result in fatal accident. Also this is against the ILOregulation of confined space entry procedures.



Observation # 18 regarding High Noise

It was observed that workers were working at maintenance shop without any ear protection although area was marked with 110dB as High noise area. Continuous exposure in high noise can cause serious health problems for workers. Also it is against the ILO regulation of noise control.

Conclusion Occupational health and safety is a subject which needs to be managed properly. Failure to this might result in legal notices. This report summarizes all the issue which need your attention and prompt action for resolve their issuers to avoid any mishap/accident in future. Accident cause pain and suffering not only for the victim itself but also for the friend and family members of the victim. An accident free environment is highly desire in each and every industry. Recommendations: Recommendation Observation # 01: It is highly recommended to place at least 10 nos of DCP-25 Kg, 08 no’s of 12 Kg & 02 no. of 50 Kg fire extinguishers Observation # 02: It is recommended to provide purified water and also install cooling/purifying dispenser unit.

Recommendation Observation # 09: It is highly recommended to calibrate all the sensors timely. Furthermore a procedure to be established to check all the detectors periodically for calibration. Observation # 12 : It is highly recommended that the worker cleaning the sludge should be provided safety harness/ lanyard for escape. Secondly it is also recommended to provide breathing apparatus with an attendant supervising the job. Observation # 18: It is highly recommended to provide ear protection like ear muffs and plugs to all employees working at site.
