12 IN ORDER TO ESTABLISH PROPER LEVEL OF COVID-19 PREVENTION THE FOLLOWING MEASURES SHALL BE IMPLEMENTED AND ASSURED: Limiting the time an employee spends in a hot environment by means of the work/rest cycle program. Reducing the metabolic demands of the job through mechanization, use of special tools, or increase in the number of employees assigned to the task. Raising heat tolerance by means of the heat acclimatization program. Training supervisors and employees to prevent heat stress conditions, recognize early symptoms of heat illnesses, and first aid procedures. Implementing a buddy system in which employees observe fellow employees for early symptoms of heat stress such as weakness, unsteady gait, irritability, perspiration levels, disorientation, or changes in skin color. Providing adequate amounts of cool 50-59 degrees F (10-15 degrees C) potable water in the work area and instructing employees to drink one cup of water every 15 to 20 minutes.. 8 Ensure that all procedural requirements are identified and fully adhered to. Conduct Pre-task briefing and brief crew on scope of work, approved mobilization, demobilization and movement route for equipment, potential emergencies, dos and don’ts during advent of an emergency, location of muster points, emergency contacts, hazard associated with task, control

measures put in place & residual risks Site Induction for all crew members including operator and drivers is mandatory Only qualified and TP certified operators and drivers be allowed to operate equipment / vehicles Proper inspection of all equipment (Equipment should good condition with spark arrester, reverse alarm, fire extinguisher) be ensured prior to entering the facility. All equipment must hold valid Inspection and TP Fit for Service Certificate Route for Mobilization and Demobilization of equipment be approved by CPY. Exit route and safe area to be discussed as part of pre task briefing. Conduct a survey of the route; check the facilities which are at high risk (overhead power lines, light poles and other structure) Moreover, Check ground stability of the track which will be used for movement of equipment Housekeeping-ensure routes are clear of debris/materials. Provide Trained Banksman (Banksman should be in visible jacket, ensure that operator is strictly following the Banksman signals, Ensure Banksman keep himself in visible position to operator maintaining a safe distance safe distance, Speed limit according company rules 5km/h inside facility area, all heavy traffic to be accompanied/controlled by a trained banksman during movement inside facility area. Stop movement/activity when eye to eye contact of operator is lost with the banksman Conduct wind speed test, lifting operation not allow if wind speed is more than 10 m/s and make sure that no rain and/or worse weather conditions will impact

Ensure that all emergency contact numbers are noted down: COSD +964-780-917-77-55 Ensure that all procedural requirements are identified and fully adhered to. Conduct Pre-task briefing and brief crew on scope of work, potential emergencies, dos and don’ts during advent of an emergency, location of muster points, emergency contacts, hazard associated with task, control measures put in place and residual risks All personnel shall take HSE induction/orientation from CPF HSE. All personnel shall have a valid H2S awareness training [delivered by an approved TP training service provider] Only authorized persons be allowed to enter into worksite Apart from task specific PPEs, all personnel shall have mandatory PPEs such as Hardhat, Safety Goggle, Safety Shoe, Coverall, Personal H2S Detector and suitable face covering [Bandana/mask]. Ensure that workers are equipped with half mask respirators to escape a conceivable dangerous environment [emer. escape mask] with an MUC of 100 ppm. Prior to start of activity and then at regular interval Gas Testing to be conducted by Authorized Gas Tester. Only competent and qualified personnel be assigned to undertake the task

Type of potential emergencies, emergency response plan, preparations, escape route and do's & don'ts during advent of an emergency be briefed to workforce in pre-start briefing Emergency contacts be shared with workforce Ensure that First Aid Kit is available at site and sufficient number of first aiders are also present Prepare site by posting warning / awareness signs and barricades. Use Proper Manual Handling technics. When load has to be manually handled, it should be held or manipulated as close to the body as possible. Twisting turning and bending of the back should be avoided. Make sure you can see over the load. Try to push the load rather than pill. Use leg muscles as much as possible. Consider dual lifting (General rule of thumb if above 25Kg). Regular breaks should be taken. Check the route before lifting. Ensure proper access to job area. Make sure working area free from obstacle Avoid rushing while working on task. Perform the task with fully attention. More than 25kg weight shell be lifted by 2 persons Ensure that hand tools free from lubricants Ensure remove jewelry from hand. Ensure to use proper gloves. Avoid putting fingers into pinch points. Always wear proper PPE. Check access to site prior to commencing work to assess the condition. Make sure the green tag on scaffold, check the connection carefully before climbing.

Ensure that all workers are using full body Harness with double lanyard. All other personal to stay outside the barrier area when working up. Verify all hand tools are in good condition and well carried out for avoiding to fall down. Tie hand tools with hand during work at height. Competent person with proper training and skills required Proper PPE and safety harness should be used. Do not carry materials & tools while using ladder. Ensure that a safe elevation method is being used by workers. Ensure that all workers are using the 100% tie-off technique. The worker should always have three points of support to climb, must be careful and cautious. Only one person at a time must use the ladder. Clean all area if it is slippery free from tripping hazards Supervisor and Foreman check that all tools, equipment and materials have been removed or secured. Ladder should marked with current color. If wind more than 10M/S the activity should be stop. Ensure any activity involving work near area with pressurized or energized systems or simultaneous work must secure valid approval from area supervisor in charge prior to commence of activity. Ensure all workers are trained and has attend safety induction on working in Energized Area. All workers must be fully aware of this existing hazards on work area. Task supervisor must be clearly discussed to workers related hazards involve in their work area.

No any work to be done on pressurized or energized systems unless LOTO and isolation certificates has been issued and approved by department. Never touch the cables or valve without permission
Work only on specific system or lines approved. Verify system and lines on approved P and ID. Gas test to be conducted prior to entry and during work duration by authorized gas tester inside restricted areas. Do not leave any insulation materials unattended on elevated areas. Use dust mask while Performing Insulation work activities. Use the rubber gloves while working with Rockwool. Try to avoid dirty hands or gloves to contact with eye and skin. Use special gloves & coverall while Performing Insulation work activities. Use only proper PPE recommended as per MSDS. Ensure stand by eye wash and safety shower available near work area MSDS must be available on work area and clearly discuss by task supervisor to workers related hazards. Defective and home-made tool shall not be used on site.
