PURPOSE The purpose of this document is to specify in detail the required controls, working practices and legislative requirements for the safe and controlled use of all COPI owned and operated loose lifting gear both onshore & offshore via the use of installation specific rigging lofts.
APPLICATION / SCOPE This document shall apply to all COPI owned loose lifting gear both onshore and offshore including, but not limited to, wire rope slings, chain slings, chain blocks, pull lifts, fiber slings, hooks and fittings, swivels, shackles, eye bolts, rigging screws, wedge sockets, plate clamps.

The scope being to standardise the looking after of all loose lifting equipment in regular service at all COPI owned installations/premises by means of the use of installation specific Rigging Lofts and thus ensuring compliance with adopted International Standards by COPI.

DEFINITIONS
3.1
Lifting Gear or Accessories Equipment for attaching loads to lifting apparatus including, but not limited to, wire rope slings, chain slings, chain blocks, pull lifts, fibre slings, hooks and fittings, swivels, shackles, eyebolts, rigging screws, wedge sockets, plate clamps.
3.2
Rigging Loft Dedicated storage container which is specially equipped internally to safely and securely store loose lifting equipment.
3.3
Quarantine Area An area usually within the rigging loft however can be at any other designated point at the work site/installation for the secure storage of defective items of lifting equipment awaiting action.
3.4
Competent Person/SPOC A person, appointed by the company, who has the requisite practical and theoretical knowledge and experience of lifting equipment and lifting (rigging) procedures and is thus able to examine / inspect the equipment to detect any defects or weaknesses and to assess their importance in relation to the safety and continued use of the lifting equipment.
3.5
Colour Code A method of marking equipment (normally with paint) to give an immediate visual indication of its certification status. This colour s should hould be change changed d at each periodic examination (usually 6 months). Note that webbing slings of all types should not be colour coded with paint. Alternative marking systems should be found.

RESPONSIBILITIES/ACCOUNTABILITIES Vice President Operations is accountable for the Rigging Loft Management Procedure. It is the responsibility of the Competent Person/Single Point of Contact (SPOC), or his appointed nominee, at each location (offshore and onshore), to implement the procedure, ensuring proper control, dispatch and use of all lifting gear. The appointed Competent Perso Person/SPOC n/SPOC shall be fully responsible rigging loft. for the overall management, maintenance and control of the

STORAGE A secure and often portable container/box (for off offshore), shore), or dedicated specific storage area (for onshore) which contains a selection of certified loose lifting gear, for use in lifting operations either onshore or on an offshore installation or similar work site. REGISTER & CERTIFICATION A register of all loose lifting gear shall be kept and maintained and updated by the Competent Person/SPOC (or its designee) for all items in storage or in service. The register shall be available for reference and audit as and when required at the rigging loft location. The register shall contain the following information o Copy certificates of test/certification from manufacturer/supplier of all supplied lifting gear. Unique identification number.

Date of manufacture/first test.
o
Safe Working L Load oad (SWL) and proof load applied.
o

Date of last inspection and interval of examination
In addition to the above an electronic Register shall be created and maintained for the control and tracking of each piece of lifting gear at its designated installation/operation area. This shall b be e u updated pdated at reg regular ular intervals. This shall be done as required to keep the register up to date. The register at the rigging loft can be a printed copy of the electronic register. This register shall be used by the 3rd Party Inspection Contractor when carrying out the 6 monthly inspections of the rigging loft contents and shall be updated by the inspection/certification contractor upon the completion of the 6 monthly inspections. Access to the Register and required documents shall be made available to relevant persons.

All lifting lifti ng gear to be used shall have required documentation and certification, such as Certificate of Conformance/Proof Test and Certificate/report of last inspection. 8
EQUIPMENT ISSUE & RETURN
Each item “Competent lifting gear, prior” to shall be examined by the appointed “of Competent Person Person” for issue/return, any damage, correct and appropriate markings, safe working load etc. The documentation for each item shall also be reviewed, and checked against the actual item. Equipment found to be in acceptable, satisfactory condition with the correct and appropriate documentation/markings shall be released for use. This shall then be recorded in the “Issued/Return “Issued/Return Register” and detail to whom equipment issued, date of issue/return, type and quantity of equipment, equipment identification number and any other relevant comments. The User shall then received/returned item.

EQUIPMENT MAINTENANCE In addition to the equipment inspection requirements, a program of first line preventative maintenance shall be implemented. This maintenance shall include where applicable as required, the application of lubricants, rust inhibitors, post use cleaning and correct storage. All necessary measures shall be taken to maintain the equipment in a safe and serviceable condition thus reducing the risk of failure and potential injury to personnel and damage to assets. Frequency and type of maintenance will be as deemed necessary and in conjunction with any manufacturers/industry guidelines and details shall be recorded in the comments section of the lifting gear register.

DEFECTIVE / CORRECTIVE ACTIONS In the event of an item of lifting gear being found or returned defective, the Competent person shall clearly label the item “REJECT “REJECT - DO NOT USE”, colour code the item RED, state reason for rejection and place item in the quarantine section of the loft. Where an item is found during use or in service to be defective the individual(s) shall cease work immediately and return the item to the Competent person for further advice and action. If the Competent person then deems any item of lifting gear to be beyond economical repair they shall take the necessary actions to destroy or render the item non serviceable; e.g. a wire sling shall be cut in two and in the case of chain block the hand and lift chains shall be cut. It is the Competent person’s responsibility to ensure that any rejected and/or defective items do not return to service.

Details of any rejected and/or destroyed items shall be recorded in the “Rejected Equipment Register” and shall sha ll show all necessary details i.e. equipment type, serial number, reason for rejection, item scrapped, item returned to supplier for repair, awaiting parts etc. If an item is scrapped or destroyed on the platform or at site, its record should also be removed from the register of loose lifting gear. If a rejected item is subsequently deemed not repairable and is scrapped, its record should also be removed from the register of loose lifting gear. All rejected items shall be replaced where possible at the earliest opportunity. This shall be done by way of formal written request to the budget holder for the platform/site. 11
COLOUR CODES All items are to be colour coded on completion of satisfactory examination as detailed in the table below. The paint or alternative method of colour coding coding should be durable and when applied should entirely obliterate all traces of the previous colour. Webbing and round polyester slings should never be painted. Alternative means of marking these these items should be devised. devised. Colours to be used are Blue, Orange, Green and Yellow. Applied colour code shall shall only be valid for a maximum of 6 months.

IDENTIFICATION AND TAGGING All Loose Lifting Gear shall be identified with a unique identification no. clearly marked on the equipment. In addition a name plate should also be attached to the equipment showing the following information. Numbering system :
