HOW TO APPLY FOR THE EXAM STEP 1. Secure Online Online Application System from the PRC website www.prc.gov.ph make sure you have an updated CEDULA before filling out form. STEP 2.

Accomplish one pagepage-application form by filling out all the blank fields in the application form. Fields marked with a red asterisk (* ( *) are required. Submission will not be successful until all required fields are properly accomplished.

STEP 3. When the encoding is completed, click the save button, and then OK. A notification message will appear on the screen confirming that you have successfully submitted your application. A similar notification will be sent to your email ( if you provided your email address). STEP 4. Proceed to the nearest PRC Office and present all the documentary requirements for assessment, payment of prescribed fees and issuance of Notice of Admission (NOA)9 BEFORE the deadline for application.

DOCUMENTARY REQUIREMENTS: • Original and photocopies of Transcript of Records with Special Order and Date of Graduation, and with scanned pictures with remark “ FOR BOARD EXAMINATION PURPOSES ONLY”. Graduates of government schools and institutions/programs accredited by recognized accredited agencies under the FAAP are exempted from SO. Graduates of new Schools/Degree Programs must submit School Recognition and/or Permit to operate. • Original and photocopies of NSONSO-Issued Birth Certificate (if NSO is not clear, bring copy from the Local Civil Registrar) • For Married Females, original and photocopies of NSONSO-Issued Marriage Contract (if NSO is not clear, bring copy from the Local Civil Registrar) • Two (2) passportpassport-size colored pictures with white background and complete nametag. • Current Community Tax Certificate (CEDULA) (CEDULA) • Other specific requirements as required by the Commission or the Professional Regulatory Board.

STEP 5.

STEP 6.

Pay for metered documentary stamps and mailing envelope with metered documentary stamps at the CSC.

STEP 7.

Accomplish the CAF, PERRC, and NOA. Indicate the Official Receipt number, date, and amount on the CAF.

STEP 8.

Proceed to any of the Issuance Windows (Window 9, 10, 11, 12) for final review of qualifications and documents and issuance of NOA, Applicant’s Stub, Program of Examination, Examinee’s Guide and SelfSelf-Instruction Sheet.

STEP 9.

Keep the Notice of Admission, Official Receipt, and the Applicant’s Stub. Bring these with you on the first day of the examination. For further information, read the General Instructions at the back page of the Notice of Admission, Program of Examination, Examinee’s guide and SelfSelf-Instruction Sheet. 11

STEP 10. Bring your NOA when you verify with the PRC your school and building assignment which will be posted at the PRC premises two (2) three (3) working days before the examination date.

TERMS AND CONDITIONS • Previous data entries will be automatically cancelled if you have multiple data entries. • NO APPLICATION shall be accepted after the deadline. • Payment will be forfeited if you are not qualified to take the examination. • Applications submitted without payment and required documents will not be included in the official list.12
