Application Process for ELIGIBILITY ID *Applicant should submit all required documents before we start processing their application.
INCOMPLETE REQUIREMENTS WILL NOT BE PROCESSED 1. Creating Applicant’s Mumaris Account and Uploading Necessary Documents 2. SCFHS will review applicant’s account and will release Eligibility ID once application has been approved. *issuance of Eligibility ID will take 4-7 days. However, issuance of such may take longer than usual due to the following circumstances a. Wrong documents uploaded b. Document that is not clear, readable, tampered and the like c. Incomplete requirements
3. Once Eligibility ID has been issued, we will contact Applicant via SMS/Email. 4. The Applicant has to deposit the Assistance Fee of 1,500 thru our EastWest Savings Account or any Remittance Center after issuance of Eligibility ID. *However, in cases that the Applicant will also wants us to schedule their Prometric Exam, an additional Examination Fee of $90.00 or Php 4,300 shall be collected. 5. After the fees have been deposited, the Applicant shall be notified of the available Date and Time of the Exam for them to select. 6. Once Applicant decided of the Date and Time of Exam, we will schedule their Exam right away 7. Upon confirmation of Schedule of Examination via www.prometric.com, all details will be forwarded to Applicant’s Email Address. 8. Applicant have to Print the Appointment Confirmation and bring it along with their Government issued IDs on the day of their Examination.
