  1-cognitive state exam. 2-stroke risk factors.take histor only. 3-handwashing. 4-diplopia. take history and discuss dds with the examiner. 5-carpel tunnel syndrome. take only focused history, do examination and dds with the examiner. 6-postmortem councelling.get consent. 7-70 yr old lady with osteoarthritis. evaluate risks and discuss management with the patients. 8-needle stick injury with a child.councel the mother.she was concerned abt hiv. 9-child came with dka. talk to the mother and discuss management.she was unaware of the diagnosis. 10-unconcious pt.gcs and neurological exam. 11-pefr and respiratory system examination. 12-suturing. just do the task and no commentary plz. 13-rest. 14-dysmenorrhea.talk to the lady and discuss possible mgt with the pt.she did not want ocps or nsaids or any prost....... 15-testicular carcinoma.just history and examination done shows mass attached to the testis.also discuss the further management and adress his concerns. 16-thyroid examination. 17-pt has been planned for sigmoidectomy, left hemicolectomy, resection and anastomosis, explain the procedure to pt,adress his concerns,also discuss abt possible colostomy.

   

1-thyroid exam...no positive finding except for sweaty palms which i think the guy put some water just before,no tremors. he was a middle aged man. no swelling in the neck ,every body had the diagnosis of normal thyroid..... 2-child with dka...he was brought in a semicomatose state and found to have ketones, high blood sugar as well as i think abgs showed acidosis...it was the first time this has happened...so councel the mother abt the child's status and explain management....first start of with diagnosis...mother was not aware of the child's diabetes...you have to talk a little abt the immediate mgt that you have done,,,,like iv fluids etc dont go into that detail,,just adress her concerns,,she already knew wat diabetes was,since her uncle was diabeteic, she was more keen on asking abt insulin and life style modifications and i mentioned bracelet,and glucometer and a diabetic diary as well as hypoglycemia demonstration, she was happy in the end as i reassured her,she was panicking in the beginning.... 3-osteoarthritis...she was a 70 yr old woman,obese, and had a sedentry life stlye,diet was not proper and she recently had a fall and had a fracture....so just asked a couple of questions...she had already known about osteoporosis since she visited the hospital when she had a fracture before,,she was keen on discussing the management....no need for hrt here....she was 70.......mentioned about diet....vitamin and calcium supplements....weight bearing exercises....physiotherapist.....and bisphosphonates..she wanted to know details abt bisphosphonates..... 4-resp sys & pefr....the examiner just wanted to know the reading,,,not low or high,,,just wat the pointer said,,,250 for me...the pt was an old man,,had gynaecomastia and on auscultation there was ronchi on the right upper part of the chest...... 5-suturing was really funny.....it said in the task" no commentary plz" every thing was assume inside,no normal saline to flush,just one tooth forcep,antiseptic solution,gauze packs i mean swabs,,no drape...everything he said assume and plenty of time put 2 sutures...he was keen on looking at the technique....in the end i verbalized abt putting the needle in the sharps 3 times...and instuments for sterilization.... 6-needle stick---no test plz,,,y the test mother was anxious i calmed her down she was anxious abt hiv,she was convinced was easily when i said hiv does not get transfeered so easily just 0.5% chance even with a loaded syringe....i asked abt the child's immunization status and he was up to date with the jabs,,,then discuss mgt with the examiner,,just told him that discuss with seniors and give him pain killers if in pain and confirm the immunization status myself and if needed than give him a tetnus shot... 7-in diplopia it was take detailed history format...she had diplopia when looking to the right when writing on the blackboard...typical lateral rectus palsy....but you had to rule out other differentials,and all past hx,medications,allergies,personal,social,travel etc...........then dds with examiner.. 8-unconcious pt.....stupid simulator was confused abt his gcs he firt gave gcs of 8 then 11.everyone had different gcs for him..hypertonia throughout upper and lower limbs..did reflexes... upper and lower limb

mentioned abt babinski,,and pupillary light reflexes.

Manikins:

1- Ophthalmoscopy "it was proliferative retinopathy" , the examiner was really nice, and now u don't need to examine the manikin with the opthalmoscopy.. u just examine the patient for red reflex.. then just sit on the chir and look directly into the manikin eye.. really better

2- Testicular examination.. 2 solid swellings one in each testis, most probably cancer

3- CPR of a child.. the examiner was nice, she just let's u stop after u do the phone call.. but maybe coz it's the end of the day..

4- Pap smear.. with the new technique "cervical brush"

Examinations:

1- Knee examination: was medial collateral ligament injury plus mention differentials

2- heart examination of pt with heart failure

Hx and counselling

1- Hx of male 25 pt with history of shortness of breath, dry cough and weigh loss "PCP but unfortunately i messed it up.. don't know how couldn't i think of pneumonia.. the man mentioned my partner noticed i loosed weight.. looked homosexual for me but didn't want to ask about something irrelevant.. but it was obvious for everybody it was just my cloudiness who made me think about lung cancer mainly

2- Hx of incontinence for 6 years with no medications and manage "stress incontinence"

3- Hx of headache and DDs with examiner "really nice old lady and it was giant cell arteritis"

4- Mother with her child having fracture ankle.. preoperative assessment for anesthesia as he's undergoing OR and IF

5- Suicidal risk assessment of female in low mood "crying with eyes down"

6- Son of a mother with hypothermia, counsel about the incident and future management.. The sun was to be rude coz mom living alone and wasn't helping, when i told him i'll be offering your mom some advices not to live alone as she can move to a pensioner house.. he said no i'm against that!!! i was like whaaaa!

7- Counsel a patient with incurable lymphoma.. will be put on morphine.. the patient was stubborn about morphine, its side effect, he said so now i'm a terminally ill patient doctor, so am i going to die? and also he knows it's a addictive.. i reassured and couldn't believe when before 5minutes ring bell he told me ok doctor i'll accept it if it's the best option for me

8- Counsel a patient about pain relief when he developed severe pain the first day after herniorrhaphy

9- Counsel a patient who were on OCP and stopped it by her GP as he has DVT 2 months ago.. give her other options

1- Dose calculations : i missed it up coz i couldn't have a brain unfortunately.. really sleep before the exam, it's a must
