I, Jonathan De Guzman Rivera,  of legal age, single, and a resident of Brgy. Catbangen, San Fernado City, La Union, petitioner in this case, state under oath as follows: PRELIMINARY STATEMENT

 The person examining me is Atty. Franeli Jessa C.  Jaramilla with office address at San Fernando City, La Union.  The examination is being held at the same address. A:

My middle name and my last name, Ma’am. Ma’am.

The erroneous middle name is Reyes. My correct middle name is De Guzman. The erroneous last name is De Guzman. This should be correctly registered as Rivera.

What are the names of your parents?

A:

My father is Rogelio Cariaso Rivera and my mother is Jennifer Reyes De Guzman. Guzman.

Were your parents already married on the day of  your birth?

A:

Their marriage contract bears the date September 28, 1998. In fact, they executed an Affidavit of Legitimation but it was denied because that time, my father was allegedly married to another already. However, I have a copy of my father’s Certificate of No Marriage validly issued before their marriage. (Exhibit “P”)

Aside from your Certificate Certific ate of Live Birth, what other document do you have that bears your correct middle name and last name?

A:

My baptismal certificate, Ma’am. It shows here my correct name which is Jonathan De Guzman Rivera. (Exhibit “J”)

How are you known by your friends and family?

I am also known in my school, since elementary until college by my name Jonathan De Guzman Rivera. All my school records show that fact.

A:

Yes, Ma’am. My elementary, secondary, and tertiary school records like certificates of graduation and transcript of records bear my correct name. All my school ID's also show the same name. name. (Exhibits “K and series”, “L”, “M”, “N”)

How about your medical records? What name do they contain?

A:

Ma’am, I Ma’am, I brought here a copy of a medical certificate issued by Bethany Hospital. It shows here the name I am known for and the name that I have been using which is Jonathan De Guzman Rivera. (Exhibit “O”)

What are the effects of this erroneous information to  you?

A:

Such discrepancies discrepancies greatly affect my personal data and cause confusion.

In order to avoid confusion and to remedy the problem, I humbly ask the court to direct the Local Civil Registrar to cause the correction of my erroneous middle name and last name, as appearing in my Authenticated Certificate of Live Birth:
