PETITION

COMES NOW, the undersigned petitioner, Shaniz A. Abalorio, Abalorio, unto this Honorable Court respectfully states that: 1.  She is a Filipino citizen, single, over twenty one (21) years old and a resident of Bulaklakan Street, Lanozna Subdivision, Matina, Davao City Philippines, copies of the picture of the petitioner are attached herein as as   Annex “A”; 2.  She has completed her law studies at Ateneo de Davao University in June 2, 2018; 3.  She was admitted to the Philippine Bar on June 14, 2019 , a copy of the Certificate of Membership is hereto attached as Annex “B”; 4.  She is a bona fide member in good standing of the Integrated Bar of the Philippines, Davao City Chapter, attached is a copy of her Certificate of Good Moral Character as Annex “C”;  5.  She has paid her membership dues on June 7, 2019 as evidenced by IBP Official Receipt No. 088342 issued by the Integrated Bar of the Philippines, hereto attached as Annex as Annex “D”  6.  She has paid her Professional Tax as evidence by an official receipt No. 2378016 issued by Office of the City Treasurer attached herein as Annex “E”; 7.  She has attached herewith and made integral parts hereof her specimen signatures as Annex “F”; 1

 

8.  She has likewise attached the true impressions of her dry seal, as well as the official receipt of the manufacturer as  as   Annex “G”  and Annex “H”  respectively; 9.   Attached herein is an Affidavit of Undertaking as Annex “I”, wherein she undertakes to provide the Original copy of the Clearance/Certificate of Good Standing issued by the Office of the Bar Confidant; 10. She possesses all the qualifications and none of the disqualifications of a Notary Public; 11. She has not applied for commission before any other jurisdiction; 12. She undertakes to inform the Executive Judge within ten (10) days in case of any change in the given address and to submit her monthly report religiously; 13.  She now desires to be granted Notarial Commission as Notary Public for and in the City of Davao. PRAYER

WHEREFORE, in view of the foregoing, it is respectfully prayed of this Honorable Court, after due notice, publication and hearing to grant this Petition for  Appointment  Appoin tment of Notarial Commissio Commissionn of the herein Petitioner for and in the City of Davao. Other just and equitable reliefs are likewise prayed for.
