xx x x PETITION FOR DOMESTIC ADOPTION OF MINOR The PETITIONERS, through the undersigned counsel and unto this Honorable Court respectfully avers that: 1. That PETITIONERS of legal age, TEOPISTO VILLANUEVA and PAULYN VILLANUEVA, Filipino citizens, are husband and wife, residing at 1916 Joya South Tower, Joya Drive, Makati City; 2. They have no legitimate children or descendants and hereby desire to jointly adopt a minor, named FLORENCE LUCERO, eight (8) years of age, legitimate child of GABRIELLE MUNSAYAC and MATS LUCERO; 3. The father of said minor is now dead, and only her mother, named GABRIELLE MUNSAYAC, is alive;
4. The said mother, GABRIELLE MUNSAYAC, is not insane, nor intemperate and has executed a Deed of Voluntary Commitment, as evidenced by Annex “A”, hereto attached and made an integral part of this petition. 5. The Department of Social Welfare and Development has issued a Certificate of Availability for Adoption certifying that the minor FLORENCE LUCERO is available for adoption, as evidenced by Annex “B”, hereto attached and made an integral part of this petition. 6. The petitioners are qualified to adopt the said minor and have complied with the requirements under the Domestic Adoption Act of 1998 (R.A. No. 8552), and will be able to financially and morally bring up and educate said minor properly and adequately. WHEREFORE, in view of the foregoing, it is respectfully prayed that upon due notice and hearing, judgment be entered adjudging that the minor child FLORENCE LUCERO be declared for all legal intents and purposes, the child of herein PETITIONERS TEOPISTO VILLANUEVA AND PAULYN VILLANUEVA, and that her surname be changed to that of the petitioners.
