Diagnosis: Alzheimer’s disease. Symptoms began 3-4 years ago. Progress: Memory loss becoming more severe. Unable to remember basic information or routines. General mental condition deteriorating. Verbally aggressive. Sometimes physically. Incontinent of urine Wakes often during night. Difficulty to assess due to hyperactivity.

Family/Social Situation Married Lives with husband (retired police officer) Husband struggling to cope. Often tearful/frustrated at deterioration in wife’s condition. Fears for her safety. Feels impact on their social life. Reports being very tired. 2 adult sons live interstate No other family nearby

WRITING TASK

Using the information in the case notes, write a letter of referral to Angela Hope, who is a Community Care Manager of the Government Community Service. Introduce Mr and Mrs Brown and explain why you are referring them to the social worker. Discuss reasons for their situation and explain how you think Ms Hope can help. DO NOT use note form in the letter; expand the relevant case notes into full sentences. The letter should have at 180-200 words.
