That he filed an application for probation on May 19, 2016 to this Honorable Court; 2. That he is presently confined at the Capiz Rehabilitation Center, Lawa- an, Roxas City for violation of R.A. 10591 otherwise known as Comprehensive Firearms and Ammunitions Regulation Act and failed to post the required bail bond;

3. JEFFREY DELA CRUZ a responsible member of the Community, Barangay Councilor of Barangay Poblacion, President Roxas Capiz, together with JOVER BEREBER, new elected Sanggunian Bayan Member of President Roxas Capiz, both residents of Barangay Poblacion, President Roxas, Capiz, intends to accept the custody of the undersigned accused and guarantee his appearance whenever told to do so by the Court or the Probation Administration. WHEREFORE, in view of the foregoing, defendant most respectfully prays that he be released from the custody of Capiz Rehabilitation Center, Lawa-an, Roxas City.
