MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE T HESE PRESENTS:  This Memorandum of Agreement has been made and entered into this 6 th of June 2011, in Tabuk City, Kalinga, Philippines by and between: Laoagan Inn and Resort, Resort, a business establishment duly organized and existing under the Philippine Laws, with business address at Bulanao Tabuk City, Kalinga, represented by its Proprietor, Mrs. Mary Laoagan, herein after referred to as the FIRST PARTY. And CORDIL CORDILLER LERA A A+ COMPUT COMPUTER ER TECHNO TECHNOLOG LOGY Y COLLE COLLEGE, GE, a corp corpor orat atio ion n duly duly organized and existing under the Philippine Laws, with office address at Bersamin Bldg., Bldg., Dagup Dagupan an West West Tabuk Tabuk City, City, Kaling Kalinga, a, repres represent ented ed herein herein by its Presid President ent,, Ms.GLADYS MAE O. GARCIA, herein after referred to as the SECOND PARTY. WITNESSETH

WHEREAS, the state recognizes the importance of the participation of the private sector in assisting the Government in the realization of its trust in the delivery of the basic services to the people; WHEREAS, WHEREAS, business establishments aim to help and assist colleges in preparing its students and in providing the training needed as they compete in the work arena; WHEREAS, Cordillera A+ Computer Technology College, aims to strengthen the knowledge and skills of  the students acquired in their respective Physical Education Subject through undergoing actual training on First Aid and Water survival; NOW, THEREFORE, THEREFORE, in consideration of the foregoing premises, the FIRST PARTY  and the SECOND PARTY , hereby enter and bind themselves to this MEMORANDUM OF AGREEMENT and hereby undertake and willfully oblige themselves to faithfully comply with the following terms and conditions; Responsibilities of the FIRST PARTY (Laoagan Inn and Resort). 1. That the FIRST PARTY shall PARTY shall permit the use of their swimming facilities for the training of the BS Criminology Students of the SECOND PARTY during PARTY during their actual training on their PE 3-FIRST AID AND WATER SURVIVAL. 2. That th the FIRST PARTY shall PARTY shall ensure the safety of the students of the SECOND PARTY while PARTY while undergoing training through the presence of their lifeguard. Responsibilities of the SECOND PARTY (School). 1. That th the SECOND PARTY shall PARTY shall make reservation with the Proprietor of the Resort, one week prior to the use of the said swimming pool.

2. That the SECOND PARTY shall direct the students to abide by the policies, rules and regulations regarding the use of swimming pool of the FIRST PARTY. 3. That the SECOND PARTY shall pay the prevailing rate regarding the use of  the swimming pool. FURTHEREMORE, both parties agrees that; 1. That this contract will take effect on June 6, 2011, and shall remain enforce unless otherwise revoked by either party, upon due notice of at least (60) days prior to termination 2. Any breach committed by either of the parties and the students, on the provisions of the agreement shall cause the recession of the same without a need of judicial order.

IN WITNESS WHEREOF, the parties have hereunto voluntarily affixed their signatures on this 6th day of June 2011 at Tabuk City, Kalinga, Philippines.

MARY LAOAGAN AND Ms. GLADYS MAE O. GARCIA, with their respective Community Tax Certificate Numbers appearing under their names and signatures, known to me that the same is their voluntary act and deed.  This instrument consisting of________pages, including this page on which this acknowledgement is written, refers to a MEMORANDUM OF AGREEMENT duly signed by the parties and their instrumental witnesses on the left margin of the first and the _________pages thereof.

EVELYN PASCUA, CESO IV Regional Director CHED-Region II  Tuguegarao, Cagayan Dear Ma’am, Greetings! I earnestly request your good office to please extend to me your kind support to compel PO3 Ferdinand A. Datul, to sign the Approval Sheet of my Master’s Thesis at University of Cagayan Valley which I successfully defended last March 19, 2011. Considering that said Approval Sheet has already been signed by my Adviser PCINSP JOSE C. COBALLES, and the Chairman of the Panel PSUPT FE P. QUIBOL, it defies reason that I am making very costly twice weekly trips to Tuguegarao since March to obtain PO3 Datul’s signature without any success. More so that the HEI seems helpless to act on the matter where I have been reporting to the office of the VPA and the Dean which bear no result. I am constrained to ask the kind assistance of your office because my employer, C/A+CT College, has given me until May 25, 2011 to comply with the minimum requirements of a College Faculty or face termination from my job.

EVELYN PASCUA, CESO IV Regional Director CHED-Region II  Tuguegarao, Cagayan Dear Ma’am, Please extend to Mrs. Natividad A. Colalong the authority of your office to seek favorable action on her request “ to compel PO3 Ferdinand A. Datul, to sign the Approval Sheet” of her Master’s Thesis in Criminology. Mrs Colalong, by virtue of an employer-employee contract signed in June 2010 was given until March 30, 2011 to earn her Master’s Degree. In consideration of her plight, I have graciously extended the period for her to comply until May 25, 2011 beyond which she faces the prospect of non-renewal of contract.  Your kind and urgent action is requested.  Truly Yours,
