IGC 3 MANAGEMENT REPORT SITE SAFETY INSPECTION Introduction The Purpose of this report is to help and identify and correct unsafe work practices and conditions before an accident occurs. An inspection was carried out at ITD Cementation India Ltd Cochin site on 02.11.2012 at 11:00 Am. During the inspection several unsafe acts and conditions observed which are highlighted for action to be taken for a safe working environment and developing a good safety culture. Executive Summary The purpose of this was to identify hazards and implement controls through field inspection and seek patterns of hazards. The following area of concern needs management immediate action electrical, excavations, walking-working surfaces, cranes and other material handling equipment, hand and portable powered tools, and welding and cutting. Effective controls identified during the inspection are also described in this report. Some major risks and hazards are found during the inspection. They are Chance of fatality due to electrical hazards, falling objects, fire and explosion, falling from height, hazards associated with slip, trip and fall. Provide Residual current circuit breakers for electrical systems, proper barricading around the excavation, install flash back arrestors in gas cutting sets, proper earthing, etc...can control the hazards. Main Findings There are some major risks and hazards. These are 1. Electrical In some electrical panel boards there RCCB/ELCB are not used. Electrical hazards have always been recognized, yet serious injuries, deaths, and property damage and fire occur daily. Providing ELCB’s with 30 mA sensitivity for shock protection. As this is an important safety measure, we recommend periodical checking once in a month. Implementing and following a well designed Electrical Safety Program will protect employees and employers against Injury to personnel ,Increased costs for insurance and work­man compensation, repair or replacement costs, OSHA citations and fines. Cost: Legal reference: 2. Mobile scaffold There are some mobile scaffolds are used for working at height work. They are good in condition and green tag also there. Peoples are working in scaffold also. But scaffolds wheels are not properly blocked. This will cause of a severe accident. There are no adequate supervision and proper training. So this happened. If an accident occurs it will affect the organization's reputation and also legal fines approx $70,000.

3. Gas cutting work

No flash back arrestors used while doing gas cutting activity. It will cause of fire and explosion. Flashback arrestor is a safety device designed to stop a flame in its tracks. It's used to prevent flash back into the cylinders. This hazards easily rectified at little costs ( 5000) to the company. If the explosion and fire occur it will affect the large area of the work site. It will cause more fatality, business loss, reputation, compensation claims, and legal penalty under HSG regulations.

4. Walkways and Stairways During the inspection some obstructions found in walkways. Paint spillage and cable trailing in walkways and also no proper illumination in walkways. It will cause the slip, trip and fall. Walkways should be clear from any type of obstructions. These types of problems easily can rectify to provide proper trailing of cables and drainage and spillage facilities. Cost: Legal reference:

Conclusion While checking the Health and safety Site Inspection report showing several safety violations in this organization. Workplace safety rules have been disregarded and this is evident by the trip hazards around the office workplace. Furthermore, incorrect manual handling techniques could lead to life-changing injuries and the management does not provide proper training in lifting techniques. In this organization there is no annual testing of Safety Belts and Fire extinguishers. This is requirements under factories act. Finally, Fire regulations have been breached through the incorrect storage of combustible materials locked fire exits etc. If these recommendations are not followed, then your company will be put at risk of negative publicity and a bad reputation through potential court cases due to breaches of health and safety law. Additionally, civil litigation will affect your reputation and will adversely affect your financial stability. Worker absence will lead to an increase in costs of re-training and recruiting new workers to replace injured workers. Furthermore, if management makes no effort to improve health and safety, the morale of the workers, and consequently the production efficiency, will suffer too.
