1.0 Purpose / Scope The document is intended to establish a system of Job Safety Analysis for all nonroutine activities and those routine jobs with high risk. The requirement of Job Safety Analysis is to support hazard identification and precautionary measures for safe execution of the job. Accordingly the focus of JSA is to examine the job against the potential hazards & precautions required rather than the exercise how to accomplish the job. The purpose of this procedure is also to define the roles & responsibilities of the personnel accountable for work planning, worksite preparation and safe job execution. The procedure is also aimed to identify the competent persons who will conduct or review JSA visualizing potential hazards and incorporating precautionary measures accordingly.

2.0 Definitions 

Job Safety Analysis: Job Safety Analysis is a tool to analyze each job by breaking down into a sequence of steps / tasks identifying potential hazards as well as assessing the degree of risk against each step and accordingly recommend precautionary measures to control the risk before the job is performed. It is also referred as Job Hazard Analysis.



JSA Sheet: JSA Sheet is a document which lists potential hazards at each step of the task / activity and corresponding precautionary measures to accomplish it safely.

3.0 General Requirements 

4.0 Key Responsibilities 4.1

Permit Applicant / Controlling Team 

He is responsible to develop as well as conduct Job Safety Analysis for any task or activity (non- routine activities as well as those routine jobs with high risk) which shall be carried out either by him personally or by his subordinates including contractors on his behalf.

He will conduct JSA in coordination with Permit Issuer and Worksite Supervisor for the job to be performed under permit to work.



He will sign on the JSA Sheet which shall be available at site in addition to original copy of work permit.



He is also responsible to review & update JSA time to time reflecting lessons learnt, equipment up-gradation and procedural change.



He will ensure that JSA have been accomplished for all critical / high risk activities to be carried out under his responsibility including those by contractors on his behalf



He will ensure that the control measures mentioned in the JSA are implemented at the worksite and understood by workers prior to starting the work and verified periodically through site visits

Permit Issuer / Asset Owner 

He is responsible to review the JSA Sheet submitted by Permit Applicant for the task or activity to be carried out under PTW requirement.



He is also responsible to develop as well as conduct Job Safety Analysis for any task or activity (non- routine activities as well as those routine jobs with high risk); which shall be carried out either by him or by his subordinates including contractors on his behalf.



He will ensure that JSA have been accomplished for all critical / high risk activities to be carried out under his responsibility including those by contractors on his behalf



He will ensure that all precautionary measures as indicated in JSA Sheet are in place before authorizing the work execution.



He will also review JSA prior to endorsing the permit on shift change.

Worksite Supervisor 

He will ensure that that all tasks or activities (non- routine activities as well as those routine jobs with high risk) to be carried under his supervision are provided with specific JSA



He will ensure the compliance of all precautionary measures at each step of work execution as specified in JSA Sheet.



He will keep /attach JSA Sheet in addition to the work permit copy at site.



He will brief about JSA to workers thru daily tool box talks & ensure that all workers are familiar with JSA before commencing the work.

Directorate HSE Team 

The respective directorate HSE Team is responsible to inspect the work location time to time verifying the implementation of JSA for any task / activity being carried in their area of jurisdiction.



In liaison with HR Team, will ensure carrying out training / awareness on JSA targeting KOC / Contractors employees having roles & responsibility as Permit Applicant, Permit Issuer, Worksite Supervisor & Safety Officer.

HSE Group 

The corporate HSE Group is responsible to conduct periodical audit verifying the level of compliance for JSA Procedure in each directorate.

Directorate Human Resource Team 

The respective Directorate Human Resources Team is responsible to conduct training & awareness on JSA targeting the KOC & Contractors employees having roles & responsibility as Permit Applicant, Permit Issuer, Worksite supervisor & Safety Officer.

5.0 Procedure 5.1 Selection of Task for JSA Any task or activity that has potential for hazard is an item / candidate for Job Safety Analysis. However any task or activity to be carried out under work permit authorization or any activity that is indicated in the Risk Register is more justified to be considered for JSA. Additionally the job with possibility of harmful exposure, potential of injury / illness or equipment / facility damage shall have priority for JSA. Similarly routine jobs also which carry high risk shall require JSA Normally task to be selected taking into account the following criticality and priority:       

Accident / injury frequency or severity history Potential for severe injury New jobs Modified jobs Infrequently performed jobs Near-miss frequency Employee feedback

The task selected shall be divided into basic job steps in order to determine the risk involved and precautionary measures required to mitigate the risk. 5.2 Basic Job Steps Each job consists of a set of movements or steps, which has to be completed for accomplishing the task. The primary part of JSA is to break the task into sequence of steps. Each of the steps shall be an important part of the job to be accomplished and to be kept in the correct sequence. 5.3 Potential Hazards It is highly essential to identify potential hazards associated with each step of the task to be accomplished. Every possible source of energy (such as electrical, mechanical, hydraulic, pneumatic) damage as well as injury must be identified. It is required to look the entire work sequence & location to determine every possible hazard that might exist. It is to note that the effectiveness of JSA is highly dependent upon competency & skill of personnel involved in carrying out JSA and workers executing the task. Accordingly the Controlling Team / Asset Owner must ensure that the personnel involved in carrying out JSA and executing the work have required competency & skill to identify as well as control hazards associated with the work. 5.4 Precautions Based on the sequence of steps and potential hazards associated with each step, the precautions shall be decided to eliminate, control or minimize hazards that could lead to accidents, injuries, damage to environment or possible occupational illness. Each precaution shall correspond to the job steps and identified hazards.

It is to note that JSAs for jobs that involve the use of chemicals should include a review of MSDS sheets as a part of JSA. 5.5 JSA Template A blank JSA Template is attached as Appendix – 1; which can be used as a format to record & carry out Job Safety Analysis for a particular task or activity. Additionally Safety Team has developed various sample JSA Sheets covering most common activities being carried out. These sheets are available on Safety Team webpage for ready reference, review and use. Similarly JSA Sheets for other activities having potential hazards must be developed by the Permit Applicant / Controlling Team / Asset Owner in accordance with their roles & responsibilities defined in this procedure. 5.6 Control & Distribution of JSA Sheets Once JSA is completed, it is attached to the original copy of work permit available at site. On completion of the job, the JSA Sheet shall be retained in file with Permit Issuer as a record. However for routine activities being carried without permit; JSA Sheet will available at site with Worksite Supervisor during work execution; and after work completion it will be kept in record with Controlling Team. 5.7 Modification of JSAs In case of HSEMS procedural change, equipment up-gradation, change in job or process, JSA will be modified to incorporate the change accordingly. The aim of JSA modification is to incorporate the change and address the hazard arising due to such change. It is also to ensure that work is done consistently and improvements are documented as well as implemented. The periodical review of JSA ensures its update and continued effort to control workplace hazards.
