All diving personnel adequately trained (PDS)
All vessel relevant personnel adequately briefed (PDS) Only competent & experienced personnel to do cutting (PDS) Tool Box Talk- make sure the following have been considered (as applicable) (PDS): Material to be cut is under tension? Are there any pockets that could hold unburned gases? Can vent holes be cut? Have the vent holes been cut? Are there any flammable/ explosive materials/ substances adjacent to the worksite? Is any protection for installations or fittings required? Are protective rubber sheets in place to prevent accidental arcing? Does the diver/ burner have a steady platform to work from? Are sea state and visibility adequate for work? Are any additional precautions necessary? If yes, please list them at the end of this JSA. Ensure work permit is in place (PDS) Make sure all equipment certificates are in date (PDS) Make sure the welding checklist have been reviewed and all items checked OK (operator) Use sufficient personnel and follow safe lifting procedure (PDS) Ensure welding machine is in D.C. current mode only. Check all leads and connections for good condition. Check knife switch is tested! (operator) Follow safe use of electrical equipment procedure (operator) Ensure good housekeeping techniques are always in use (PDS)

MANDATORY MEASURES BEFORE PERFORMING ANY WELDING: Next table to be read out by supervisor to diver, immediately before welding:
1.
2. 3. 4. 5. 6. 7. 8.
9.
TASK Diver and supervisor have both read and understood the welding task plan and the relevant risk assessment. The diver has adequate PPE/ work equipment- hard hat, visor/shield, rubber gloves, drysuit - according to applicable requirements. The diver is placed securely, comfortably and must ensure that he is not between the torch and the earth clamp or standing in a bight of cable. Ensure that the diver checks above him to see that there are no pockets where gases might collect and ignite. Note gases may also collect in mud and under certain conditions the mud may already contain methane, another inflammable gas. Diver must check to ensure that no molten slag or cut offs will drop onto him, his umbilical or other equipment. Welding area must be properly prepared, skimmed and grinded. Never turn the power on unless the diver asks for it and is ready to start welding immediately. Ensure that the diver never has the power on when he is changing electrodes, when he leaves the worksite, returns to the basket or comes to the surface.
