Potential Hazard 

Control Measure   

Obtain correct Permit to Work from eTEC Only authorized person allowed All personals must be TAMIMI-ISD and ETEC inducted

 

All personnel to be project and zone inducted Ensure all approved controls and prevention measures are discussed and JSA discussed with all persons involved in the work. All personnel to attend daily toolbox talk prior to the commencement of work. All personnel should be wear proper PPE under TAMIMI-ISD PPE requirement

4 .Lifting area preparation

Potential Hazard        

Improper access Personal injury Uneven ground pavement Bad housekeeping Scattered materials Slip, trip Un authorized entry Excavations and trenches

Control Measure            

Area should be clear from any debris Material must be segregate Housekeeping must be done prior to start the job and finished the job Adequate access. Ensure area is properly prepared for job Ground pavement must be leveled properly Provide proper barricades Provide signage Provided spill tray for flammable oils Provide adequate fire extinguisher Follow the monthly color code Maintain safe distance from Excavation and trenches.

Control Measure           

   

Pre inspection of crane equipment prior to start the job. Ensure crane is third party certified Follow safe operating procedures Proper supervision from immediate supervisor Prepare proper signage and barricade prior to lifting job Use PPE’s as per TAMIMI-ISD requirements Ensure that over head object has properly place and tied off Material should be properly stacked Pipes must be in proper place ,and must be in pipe racks Good house keeping If working at height must be undergo training of working at heights Use fall protection gear Proper segregation of pipes as per sizes Ensure proper fire extinguisher is available in the area Don’t put the hands in between two pipes/objects Ensure sling is third party certified Inspect the sling thoroughly prior to use Ensure lifting gear has third party certified and valid Ensure lifting gear has no signed of damage, if damage found don’t use it or replace immediately Ensure out rigging pad available and no defects Follow monthly color code Operation vision must be clear Proper communication between operator and Rigger.

      

Ensure crane must third party certified Only competent rigger allowed to performed the job Only qualified crane operator can performed the job Ensure the ground pavement level properly Provide proper barricade Provide proper signage Ensure clear visibility between rigger and crane operator

Provide oil spill tray Ensure outrigger pad is free of damage Ensure proper sling position Inspect lifting gear prior to use Lifting gear must third party certified Regular inspection of lifting equipments prior to use Lifting tackles must have a third party certified and valid Use PPE’s as per TAMIMI-ISD requirements Don’t put hands in between two objects/pipes Ensure proper access Use two tag lines No people should stand beneath the lifted object/pipes Maintain safe distance from lifted objects/pipes Be sure no overhead objects within boom range Crane load chart must strictly followed Ensure lifted pipes must be in proper and secured to avoid rolling of pipes Use wooden wedge block No person should stay within boom range Never sit, stand, walk, work under suspended load Ensure no hydraulic leakage of crane Maintain safe distance from DSL cables box bar

Wire rope shall be provided properly identified for SWL and manufacturer. Inspections shall be done by authorized personnel and by the safety dep’t. Shall keep the records. Safe working load stamp shall provide by the manufacturer Only trained persons may use electric or pneumatic tools.

Defective tools or equipment shall not allowed to use Ensure proper rigid mounting Ensure to use PPE Stay away from lifted loads Lift the pipes slowly Provide sufficient access as possible Be sure not to over load the capacity of chain block Be sure not to over load the maximum strength of wire ropes Inspect chain blocks before use All chain blocks should be 3rd party certified Use two tag lines to control load movements Use fall protection if working at height Only qualified personnel allowed to do the job Barricade the area Provide proper signage all lifting tackles must be 3rd party certified

Control Measure  

Arrange/ stage all the material in neat orderly manner. Remove damage, banding, crate material, nails etc. and dispose of properly

  

Provide trash for work areas means for maintaining. All containers should be labeled Clean the spill area immediately, and reporting of incident and spillage. Stay away from EOT crane. Maintain safe distance from live box bar

 
