GENERAL NOTES

A

Valid & complete work permit (including supporting permits) must be applied for the said activity

B

Toolbox Talk is to be perform before start work. A specific toolbox talk on work instruction (including hazards & control measures) to be conducted by the PTW Holder

C

PPE (including the additional in the JHA) shall be worn at all times in performing the work

D

All equipments must display a valid SMDS inspection sticker

E

All jobs must be closely supervised and contractor's PTW Holder must be on site at all times

F

Set up barricades around work area and display warning signs

G

Valid vehicle entry permit is required for entering into the site

H

Vehicle to exit immediately to designated parking area after delivery of material

I

Maintain good housekeeping daily at all the workplaces

J

No smoking, except at the designated area

No drug or alcohol usage are allowed

On hearing fire alarm siren, stop all works, machinery & source of ignition and walk on foot to the nearest muster point

Report any incident or accident at workplace to the Supervisor or HSE Personnel
