Pre- start job to be held daily. Pre-dive briefing before each dive. Diver should aware of their surrounding area.

LOGISTICAL REQUIREMENTS Equipment:

SRP-007/SRP-12, IMCA certified Dive Team – All divers must possess IMCA recognized certification.

SAFETY POINTS NOTE: Following the work specific JSA and identifying work scope related safety issues. On Site: JSA to be read through and fully understood by the dive team and operators before signing. Toolbox talks to be held daily. Clear dedicated line of communication between the diving Supervisor and plant operator.

DIVERS WORK SCOPE NOTE: Divers to be sure they have read and understand all the safety points set in the JSA. ONSITE: SLOPE RECTIFICATION: The Diver enter in water from the diving ladder once all pre-dive procedures have been completed and only after instructed by the Diving Supervisor The diver leaves surface and proceed down the slope until he is on specific work location. The diver guides the excavator into position from safe distance of at least 3 meters. During the operation diver must ensure his umbilical is clear and free of entrapment. To fill identified void the diver must guide the excavator to the required area before instructing the operator to place the load. For the diver to remove large rocks from the slope he must secure a rigging belly band around the rock. The diver must move clear of the area but maintain a visual of the rock whilst instructing the Supervisor to remove the rock. The diver can cover all area to the full extent of his umbilical The diver shall exit the water via dive ladder. ACCROPODE INSPECTION The Diver shall enter in water via a Ladder. He swim along the accropodes till specific location and preventing his umbilical from fouling The inspection shall only begin when instructed by Diving Supervisor On completion of the inspection diver shall leave bottom and exit from water as per instruction by supervisor via a Dive ladder. 5 dives commencing during a 12 hour working shift.

CONTINGENCY PLANNING NOTE: As set out in JSA. There is a decompression chamber on site and standby all the time on sarb-2. Diver to report any side effects immediately to the Diving Supervisor.
