AFFIDAVIT OF SELF-ADJUDICATION BY SOLE HEIR I, MIRA LOADA ALFANTE, Filipino, of legal age, widow, with residence address at Block 1, Lot 2, Narra Street Woodsgate Subdivision, Camp 7, Baguio City, Philippines, under oath, depose and say:

1. That I am the surviving wife and only sole heir of the late ROBERTO D. ALFANTE there being no other ascendants, descendants or collateral relatives, surviving, surviving, who died died intestate in the Baguio City last June 27, 2011; 2. That at the time of his death, he left the followings, to wit;

Parcel of land and all existing improvements thereon, covered by Transfer Certificate of Title No. T-83124, issued by Register Register of Deeds for City of Baguio, with an area of TWO HUNDRED FORTY FIVE (245) SQUARE METERS, METERS, more or less, located in the City of Baguio, Philippines, particularly described as follows;

That the deceased left no debts or obligation; 4. That the above-.mentioned deceased died intestate, without leaving a will; 5. That pursuant to Rule 74, Sec. 1 of the Rules of Court, I hereby adjudicate unto myself the above-described parcel of land including all improvements built thereon and Accounts by means of this affidavit and hereby files the same with the Register of Deeds for the City of Baguio, Banco de Oro SM Branch, Baguio City and other concern agencies with the request that said adjudication be made effective without judicial proceedings as prescribed by the aforementioned Rules of Court ; 6. I am executing this instrument, to attest to the foregoing facts and circumstances and for all legal intents and purposes it may serve.
