Affidavit of Explanation

AFFIDAVIT OF EXPLANATION I, ROSEMARIE AQUINO ALPERTO, of legal age, married, Filipino, with residence and postal address at Brgy. San Juan, Alaminos, Laguna after having been duly sworn on oath in accordance with law, hereby depose and say that: 1.

I was born on January 11, 1972 at Carigara, Leyte to Sps. Carina Caposano and Diego Aquino. Attached herein is a copy of my NSO Birth Certificate duly marked as Annex “A”;

On July 21, 1990, I married Jayson Caeg Alperto at San Pablo Conservative Baptist Church, San Pablo City, Laguna. Such marriage was registered on Page 74 Book No. 40 of the Register of Marriages in the Office of the City Civil Registrar, San Pablo City. Attached herein is a copy of my Local Marriage Certificate duly marked as Annex “B”;

Recently however, I discovered that there has been a discrepancy in our marriage certificate indicating our age at the time of our solemnization. My husband’s age was indicated as 20 years and 8 months and mine was 18 years and 5 months instead of 20 years and 9 months and 18 years and 6 months. Attached herein is a copy of my husband’s NSO Birth Certificate duly marked as Annex “C”;

Moreover, the Office of the City Civil Registrar of San Pablo City required me to present my Baptismal Certificate as additional requirement. However, I cannot secure such copy because our church in Carigara, Leyte was completely washed-out by Typhoon Yolanda;

I execute this affidavit to attest to the truth of the foregoing and as a requirement for the correction of our age in our marriage certificate before the Office of the Civil Registrar and the National Statistics Office.

I further execute this affidavit as a requirement for my application in abroad before the Israel Embassy.
