I, LILY BETH B. MALLARI, Filipino, of legal age and capacity, residing at 1202 Primero Tibag Tarlac City, after having duly sworn in accordance with law, hereby depose and state, to wit: 1. That I am the bona fide owner or proprietor of Business with registered name of RIZLEA GENERAL SERVICES AND TRADING. 2. That I am executing this Affidavit of CLOSURE in my said capacity. 3. That I execute this affidavit of CLOSURE to formally close down my said registered business which have already stopped its operation since the month of SEPTEMBER year 2012. 4. That since the period of registration of my business there was no amount received to be recorded as either gross or net profit. That there was no amount received as payment for the operation of the business. 5. That such business registration is intended to be in congruent with another business opportunity which on the other hand have also failed therefore no transaction transpired with regard to my registered business. 4. That said CLOSURE of business was due to no sale or no transaction 5. I am executing this affidavit therefore for all legal intents and purposes with regard to the closure of said business. AFFIDAVIT OF LOSS

I, LILY BETH B. MALLARI, of legal age, married to ENRICO D. MALLARI, Filipino citizen, and a resident of 1202 Tibag, Tarlac City, Philippines, after being sworn to in accordance with law, depose and state: That I am a duly-licensed owner of Business with registered name RIZLEA GENERAL SERVICES AND TRADING in accordance with pertinent laws of the Philippines in consideration of business registration, rules and regulations, and was issued the corresponding documents accordingly: 1. DTI Certificate with no. 01786712 (valid from July 11, 2012 to July 11, 2017 2. Mayor’s Business Permit and the corresponding Business Plate with registered no. 4361 3. 1. BIR Certificate of Registration Registered under my name; Lily Beth B. Mallari With Tax Identification no. 203-067-856-000 2. Registered Unused Receipts, Sales Invoice and Delivery Receipts That when we vacated the place we designated to be our office at Sitio Tanpoco Barangay Matatalaib, Tarlac City, said Documents or proof of registration and Certificates were misplaced and got lost. That despite diligent search and efforts to locate my said Documents, I could not find the same such that I now believe that it is now lost beyond recovery;

As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing and to support my application for the Closure, Cessation or Cancellation of the business Registration and this affidavit be used for all legal purposes and intents it may serve in the process of the said closure of my supposed business.
