AFFIDAVIT OF BUSINESS CLOSURE

I, JACQUELINE GLORIANI DE TAZA, Filipino, of legal age and with residence and postal address at 44 Mapagsangguni Street, Sikatuna Village, Quezon City , after having been duly sworn in accordance with law, hereby depose and say: 1. That I am the legitimate owner of De Taza’s Snack House located Pascual Poblete Street, Brgy. Gombalza, Naic, Cavite; 2. That such business has not been operational due to its closure last November 15, 2016; 3. That such business has been closed because of the expiration of the lease contract; 4. That I am executing this Affidavit to attest the truth and for whatever legal purpose this may serve and specifically for compliance purposes as required by the Business Permit and Licensing Office of the Municipality of Naic, Cavite.
