Procedure for Registration/Renewal of 3rd party Inspection, Testing, Training & Certification Agencies/Consultants 1.0

PURPOSE

The safety and integrity of all equipment is essential for safe operations. To enable the safe operations of such equipment in Dubai World/ PCFC jurisdictional areas, all Inspections, Trainings and Testing agencies/consultants shall need to be pre-qualified and registered with EHS (even if certified on Federal/State levels). 2.0

SCOPE

EHS Registration/Listing of Approved Agencies for Inspection, Testing and Certification of Lifting Equipment, Pressure Vessels and Fire Equipment, and Approved Agencies for First Aid and Fire Fighting Training & Certification within PCFC / Dubai World Jurisdictional areas. 3.0

PROCEDURE 3.1 Agencies (for New Registration) shall require to complete the attached form (3rd. party Prequalification form for testing Agencies) and those intend to renew the EHS Approval period shall require to fill the attached check list (IOF-6b, submission related to Lifting Equipments, Pressure Vessels and First Aid Training, and IOF-6c, for submission related to Fire Training & Fire Protection/Detection System) covering all details and submit the same in soft copy only (02 sets i.e. 02 CD) for review/comments and/or Approval to Manager – FZ & IO of EHS-Trakhees. Labeling on the CD ( not on the CD Cover) should contain the information as per the attached annexure-1 such as Purpose of Submission; Submission; Client Name; E-mail ; Facility Location; Location; Emirates; Date; EHS Ref (if any in  previous); Client letter Ref etc. 3.2 Submission of documents shall be in accordance as per the checklist (IOF-6b, submission related to Lifting Equipments, Pressure Vessels and First Aid Training, and IOF-6c, for submission related to Fire Training & Fire Protection/Detection System). Following Certificates/Documents are mandatory for submission as an initial supporting document along with the prescribed EHS Form duly filled: • Dubai Municipality (DM) Registration (Local Order 52/1990) – For Lifting Equipments and Pressure vessels • Accreditation Certificate obtained from Dubai Accreditation Centre (DAC) - For Lifting Equipments and Pressure vessels • Ministry of Health (MOH)/Dubai Health Authority (DHA) and Center for Ambulance Services for Training Centre and Instructors – For First Aid Training • Certificates of approval with validity from Dubai Civil Defense for Fire Protection and Detection system should be submitted. 3.3 Premises/workshops may be inspected and verified by EHS for adequacy prior to Approval of Certification and also at any time during the year, where deemed required.

3.4 EHS is satisfied (based on the requirements) with the competence of the 3rd party, a Certificate of Approval shall be sent to the Agency within 6 days after satisfactorily completing all the requirements. The Agencies should ensure that renewal/registration documents are sent to EHS well in advance of the expiry date of existing EHS approval. 3.5 Failure to perform as per the expectations at Federal/State or DW levels shall result in removal from the List of EHS’s Pre-Qualified Testing & Certification Agencies. This action could also be linked to (but not limit itself to) issuance of erroneous reports, in competency or failure of equipment soon after certification by the agency. 3.6 Pressure Vessel Testing/ Certification. The agency shall be qualified, experienced and competent to evaluate the mechanical design of pressure vessels in accordance with the ASME Code Section VIII. They will also be able to develop a pressure vessel maintenance program based on API 510 and other industry practices, and then apply these requirements to typical pressure vessel maintenance situations. Understand how to apply API RP 579 “Fitness for Service” and API RP 580 “Risk Based Inspection” Lifting Equipment. 3.7 PAYMENTS Processing fee of AED 8,000/ for each New Application (Lifting Equipment, Pressure Vessel, Fire Protection/Detection Equipment, First Aid & Fire Fighting Training) – payable by cash or cheque addressed to “PCFC - Trakhees” will be required to be paid as the fee for the certification, which will  be valid for one year from the date of 1st Approval. New Application shall be submitted along with  payment receipt of AED 4000/ (non-refundable) as review fee and upon receipt of provisional approval from EHS, rest of AED 4000/- to be paid for obtaining final approval. Renewal fees of AED 4,000/ every year will be required to be paid for all above mentioned certification.

3.8 RENEWAL 3.8.1 Renewal submissions shall be made one (1) month before the Approval expiry date. 3.8.2 Renewal submissions that are not made within twelve (12) months after the expiry date will fall under new submission category. 3.8.3 Name of the third parties will be taken from the website display if the renewal process is not completed within one month after expiry date of previous approval. 3.9 AMENDMENT Any changes (extension in scope of approval), turn-over and future developments within the company that has bearing with the coverage of the certificate/original approval shall be communicated & updated by submitting the required information for evaluation and further decision(s) accordingly. Payment of AED 3000/- (Three Thousand Dirham) shall be made to EHS Finance prior to issuance of amended certificate of approval.
