Perform all necessary precautions for safety and environmental issues prior to beginning work on bop assembly.

Clean the BOP body, bonnet and rams. Remove all traces of wellbore fluids. 7 Inspect entire unit for damage, hydraulic leaks or missing nuts and bolts. Replace/repair as needed. 8 Inspect BOP flange ring grooves for damage. 9 Inspect BOP box and pin quick union threads and sealing surfaces for damage. 10 Visually inspect BOP body ring grooves, ram bores, high pressure ports and bonnet face seal areas. 11 Inspect all stud threads for damage 12 Remove the equalizing valve assembly and inspect the stem and seat. Replace if damaged or worn. 13 Inspect 3/16 holes in each equalizing valve and verify complete passage is free of debris. 14 Inspect complete bonnet assembly. 15 Inspect complete pipe rams assembly. 16 Verify that pipe ram inserts and body are matched to the correct Tubing Size tubing size. Document tubing size. 17 Inspect complete shear rams assembly. 18 Verify with technical manual shear blades and body are matched. 19 Verify shear blades condition. 20 Inspect complete slip rams assembly and slips 21 Verify that slip ram inserts are matched to the correct tubing size. Tubing Size Document tubing size 22 Inspect complete shear seal rams assembly. 23 Verify that shear blades and body are matched. 24 Verify with technical manual that correct part numbers of shear ram parts are being installed. 25 Inspect complete pipe slip ram assembly 26 Verify that that ram inserts and body are matched to the correct Tubing Size tubing size. Document tubing size.

BODY SYSTEM (cont) Visually inspect all piston rod assemblies Visually inspect all cylinder assemblies Visually inspect and check the non-rising stem and threads and sleeve nut operation on threads. Visually inspect non-rising stem sleeve nut. Visually inspect the hydraulic cap. Visually inspect for damage on all indicator rods Visually inspect front and rear seals. Disassemble, clean, inspect and re-dress all flange valves. Lubricate all bores and quick union seal surfaces. Inspect all elastomers for any damage. Replace any elastomer or WM -G L-HAL-WI -502 seal found damaged. Us WM for inspection of elastomers. 37 If a drill cross is attached to the bop assembly, disassemble drill cross, pressure wash inside, inspect and make any necessary repairs prior to assembly. 38 Verify that identification/inspection band is present and current. ST-GL-HAL-WI-501 Use HMS Standard for inspection information. 39 Verify the service pressure color band is intact. Use HMS Work WM-G L-HAL-WI -501 Method (WM) for pressure band information.

HYDRAULIC SYSTEM Inspect all hydraulic hoses. Inspect for leaks, damaged or worn hoses. Replace any hose that is damaged or worn. Inspect all hydraulic adapters. Inspect for leaks and damage. Replace any hydraulic adapter that is damaged or worn. Inspect all hydraulic quick disconnects. Inspect for missing O-Rings and leaks. Inspect for any damage. Verify that all quick disconnects hold pressure without leaking. FUNCTION TEST After assembly hydraulically actuate all rams to assure correct operation. Verify all actuator indicator pins correctly indicate BOP open/close position. Verify no hydraulic leaks. Use HMS Work Method (WM) link for instructions to perform WM-G L-HAL-WI -503 Function Test on BOP's. Record results below. Shear Ram Manual Locking System Test Slip Ram manual locking system test Shear Blind Ram Locking System Test Pipe Slip Ram Manual Locking System Test If shear test was conducted install new cutter blades after the pipe is successfully cut to ensure the blades will be new when rigged up on well.

PRESSURE TEST CH 51 Use HMS Standard link for instructions to perform pressure tests ST-GL-HAL-501 for BOP's 52 Perform hydrostatic water pressure test. Verify pressure is held for 15 minutes with no leaks. ^ 53 Chart both Low and High pressure test and retain chart for records. Attach all recorded data to maintenance sheet. / 54 Low Press. Blind Ram pressure test High Press 55 Low Press. Pipe Ram pressure test High Press 56 Low Press. Shear Blind Ram pressure test High Press 57 Low Press. System/Full Body pressure test High Press LIFTING SYSTEM CH 61 Visually check lifting eyes and welds for cracks. Repair any damage. NDE all repairs and attached details of damage and repairs to this maintenance report. 62 Verify lifting slings are in good condition and have up to date certification tag on them. Re-Certify if slings are out of date. Replace if damaged. 63 Verify lifting slings are properly attached. Verify that all lifting shackles, nuts and cotter pins are vX secure and meet appropriate location and customer regulations. 64 Use link for Guideline instructions to perform Inspection of lifting BP-GL-BU-HSE-EQP0406 - Guideline devices. ENVIRONMENTAL CH 65 Dispose of all waste properly. Ensure all spills are cleaned up and properly contained. REQUIRED PAPER WORK CH 66 Check regulatory documents and decals (per State and/or Country Requirements). sS ' 67 Verify that proper company logos and decals are installed as per the Branding initiative. Verify that the SAP equipment number is clearly marked on the unit (see the ESG Brandina web Daae for Date guidelines). 68 Tag BOP to verify the size of pipe that it is dressed.
